NICU早产儿父亲疾病不确定感与产后抑郁的相关性研究

摘　　要：目的：探讨NICU早产儿父亲疾病不确定感、产后抑郁状况以及疾病不确定感与抑郁两者之间的相关性。方法选取2015年4—12月吉林大学第一医院NICU 102例早产儿父亲为研究对象，采用中文版疾病不确定感父母量表（ PPUS-FM）和爱丁堡产后抑郁量表（ EPDS）对其进行问卷调查，对疾病不确定感评分和抑郁评分进行描述性分析和Spearman相关分析。结果 NICU早产儿父亲疾病不确定感的总分为（93.85±11.11）分，抑郁总分为（11.04±2.99）分，抑郁58名，发生率为56.86％，其疾病不确定感总分（r＝0.519，P〈0.01）及其中的不明确性（r＝0.460，P〈0.01）、复杂性（r＝0.475，P〈0.01）、信息缺乏（r＝0.351，P〈0.01）、不可预测性（r＝0.385，P〈0.01）与抑郁均呈正相关。结论 NICU早产儿父亲存在疾病不确定感和抑郁情绪，且两者之间存在正相关，护理人员应采取有效措施干预，降低早产儿父亲疾病不确定感，从而改善其产后抑郁的情绪状态，提高护理质量。Objective To investigate disease uncertainty and postpartum depression status for fathers of preterm infants and explore the relationship between them. Methods A total of 102 fathers of preterm infants were selected from April to December 2015 in our hospital as research object. We used Chinese version of parent′s parception uncertainty scale-family（PPUS-FM） and Edinburgh postnatal depression scale （EPDS） to survey those fathers by questionnaire and analyzed their disease uncertainty and postpartum depression by Spearman correlation analysis.Results The PPUS-FM level of preterm infants′ fathers was medium. The total score of PPUS-FM was （93.85±11.11）;the total score of depression was （11.04±2.99）, while 58 fathers （56.86%） felt depression. The score of postnatal depression was positively correlated with the total score of disease uncertainty （ r=0. 519, P〈0.01） , with the uncertain of illness （ r=0.460,P〈0.01） , with complexity （ r=0.475,P〈0.01） , with incoherence （r=0. 351,P〈0.01）, and with unpredictability （r=0.385,P〈0.01）.Conclusions Fathers of preterm infants show a high level disease uncertainty and depression, and there is a positive correlation between them. Nurses should take effective measures to improve the disease uncertainty and depression states for fathers, and to improve the nursing quality.
