摘　　要：目的:探索头颈部肿瘤六维方向摆位误差的相关性,为提高临床摆位精准度提供依据。方法:选取30例鼻咽癌患者,使用Tomotherapy HD放射治疗系统对患者进行治疗,回顾性分析900次图像引导放射治疗(IGRT)的三维方向中左右(X)、上下(Y)、前后(Z)以及旋转方向中左右旋转(roll)、前后旋转(pitch)、顺时针和(或)逆时针旋转(yaw)的六维方向摆位误差相关性。结果:三维方向X、Y、Z均不存在相关性,Y与Z存在显著的中等负相关;旋转方向只有roll与pitch存在显著的较弱负相关;三维方向和旋转方向中X与roll存在显著的较弱负相关,X与yaw存在显著的中等正相关;Y与pitch、yaw均存在显著的较弱负相关;Z与pitch存在显著的较强负相关。结论:在头颈部肿瘤放射治疗摆位过程中应注意不同方向的相互影响关系,尤其注意前后方向、前后旋转与上下方向以及左右方向与顺时针和(或)逆时针旋转方向的摆位精确性。Objective To explore the correlation of six-dimensional set-up errors in head andneck tumors radiotherapy to provide the basis to improve the clinical setup accuracy. Methods：30 patients with nasopharyngeal carcinoma who had been treated by Tomotherapy HD Systemwere selected. The correlation of set-up errors were retrospectively analyzed in six-dimensional （Left-Right direction： X,inferior-superior direction： Y, anterior-posterior direction： Z, left-right rotation： roll, anterior-posterior rotation： pitch,counterclockwise or clockwise rotation： yaw） using the IGRT data. Results： X had no correlation with Y and Z andY had significant weak negative correlation with Z on three-dimensional directions. The roll and pitch had significantweak negative correlation on three-rotation directions. On the three-dimensional and three-rotation directions, Xand roll had significant weak negative correlation, X and yaw had significant moderate positive correlation, Y hadsignificant weak negative correlation with pitch and yaw, and Z and pitch had significantly strong negative correlation.Conclusion： More attention have been paid on the correlation of different directions, especially, the accuracy betweenZ and pitch or/and between Y and Z in the set-up process.
