75例儿童葡萄球菌性烫伤样皮肤综合征的临床表现及发病情况调查分析

摘　　要：目的:探讨75例儿童葡萄球菌性烫伤样皮肤综合征(SSSS)的临床表现及发病情况。方法:回顾性分析2012年10月至2015年7月我院75例SSSS患儿的病历资料,探讨其临床表现、发病特点及治疗过程。结果:儿童SSSS的发病以婴幼儿为多主,农村的发病率大于城市,诱因以上呼吸道感染和急性化脓性扁桃体炎最为多见,临床分型主要为泛发性。加用头孢霉素组的触痛消失时间、红斑水疱进展停止时间和红斑消失出现脱屑时间与加青霉素组相比较均明显缩短;加用头孢霉素组的治疗时间为6.61±1.01天,与加青霉素组的7.45±1.22天相比明显缩短;病程与加青霉素组相比亦显著缩短,差异均有统计学意义(P<0.05)。结论:SSSS以婴幼儿为多发,但大儿童亦存在发病的可能。SSSS可能有明显诱因,以上呼吸道感染和急性化脓性扁桃体炎最为多见。治疗时,细菌培养和药敏试验至关重要,能够有效提高治疗的效率,改善治疗效果。Objective： To investigate the clinical manifestations of 75 cases of children staphylococcal scalded skin syndrome（SSSS） and their incidence analysis. Methods： Retrospectively analyzed the medical records of 75 cases of children with SSSS in our hospital from October 2012 to July 2015, explored its clinical manifestations, disease characteristics and treatment process. Results： The incidence of children SSSS was mainly infants, the incidence in rural areas is greater than the city, the most common inducements were upper respiratory tract infection and acute suppurative tonsillitis, clinical classification is mainly generalized. The time of tenderness disappeared, erythema blister progress stopped, erythema disappeared and appeared scaling of plus cephalosporin group were significantly shortened than plus penicillin group; The treatment time of plus cephalosporin group were 6.61 ±1.01 days, which were significantly shortened than 7.45±1.22 days of plus penicillin group; And the course of disease was also significantly shortered than plus penicillin group, differences were statistically significant（P〈0.05）. Conclusions： The incidence of children SSSS is mainly infants, but has possibility of the occurrence of large children. SSSS may has significantly inducements, upper respiratory tract infection and acute suppurative tonsillitis is most common. In the treatment, bacterial culture and drug sensitivity test is essential, which is can effectively improve the efficiency of treatment and improve improve the treatment effect.
