摘 要:目的探讨不同床头抬高角度对机械通气患者呼吸机相关性肺炎和压疮的综合影响,以期为机械通气患者提供理想的床头抬高角度。方法选择ICU行机械通气的153例患者为研究对象,分为床头抬高30°组82例和床头抬高45°组71例,护理措施一致,观察两组患者呼吸机相关性肺炎及压疮的发生率。结果两组患者呼吸机相关性肺炎的发生率差异无统计学意义(P>0.05);床头抬高30°组患者压疮发生率低于床头抬高45°组患者(P<0.01)。结论床头抬高30°既能够有效预防ICU机械通气患者呼吸机相关性肺炎的发生,又不增加压疮的发生。Objective To evaluate the combined effects of elevated body position on ventilator associated pneumonia (VAP) and pressure ulcers in pa- tients with mechanical ventilation, and to find out proper height of head of bed. Methods Totally, 153 mechanically ventilated patients in intensive care unit were randomly assigned to two groups, a group of patients with backrest elevation of 30°, another group of patients with backrest elevation of 45 °. Results There was no difference in incidence rate of VAP between two groups (P 〉 0.05 ). The incidence of pressure ulcer was lower than that of 45° group (P 〈 0.01 ). Conclusion Backrest elevation up to 30 ° is a compromise between the seeming incompatible demands of the prevention of ventilator - associated pneumonia and skin integrity.
