脊髓电刺激治疗带状疱疹后神经痛的临床观察

摘　　要：目的:观察脊髓电刺激治疗带状疱疹后神经痛的临床疗效及安全性。方法:40例带状疱疹后神经痛患者,采用数字表法随机均分为两组,对照组(n=20):背根神经节脉冲射频术治疗;观察组(n=20):脊髓电刺激治疗。观察治疗前及治疗后不同时期(术后1周、术后1月、术后2月和术后3月)疼痛程度和睡眠质量的变化。采用简化Mc Gill评分(short-form of Mc Gill pain questionnaire,SF-MPQ)及睡眠质量综合评定治疗效果,并记录并发症和不良反应的发生率。结果:两组治疗后各时期SF-MPQ评分均较治疗前降低,睡眠质量评分均较治疗前明显增加(P<0.05);观察组治疗后各时期SF-MPQ评分较对照组同期明显降低,睡眠质量评分较对照组同期明显升高(P<0.01)。对照组和观察组未出现明显的不良反应。结论:脊髓电刺激治疗带状疱疹后神经痛效果佳,可迅速缓解患者疼痛症状、改善患者睡眠质量且安全性良好,疗效优于脉冲射频疗法。Objective： To investigate the efficacy and safety of spinal cord stimulation in treatment of postherpetic neuralgia（PHN）. Methods： Forty patients with PHN were randomly and evenly divided into two groups according to digital table. The control group（n =20） received dorsal root ganglion pulsed radiofrequency; the observation group（n =20） received spinal cord stimulation. Changes in pain degree and sleep quality were observed before treatment and at different time points after treatment（the 1st week, the 1st, 2nd, and 3rd month after the surgery）. The short-form Mc Gill pain questionnaire（SF-MPQ） and sleep quality scores were used for evaluation of treatment efficacy. The frequencies of complications and adverse reactions were recorded. Results： The SF-MPQ of two groups significantly decreased at different time points after treatment compared with pre-treatment（P〈0.01）, and the sleep quality score was significantly increased at different time points after treatment compared with pre-treatment（P〈0.01）. The SF-MPQ of observation group decreased significantly after treatment compared with control group（P〈0.01）, the sleep quality of observation group increased significantly after treatment compared with control group（P〈0.01）. Conclusion： Spinal cord stimulation is proved to be a good method for the treatment of PHN because it can rapidly alleviate postherpetic neuralgia, improve patients＇ quality of sleep, and also has high safety. The therapeutic effect is better than radio-frequency therapy.
