吸入糖皮质激素治疗婴幼儿轻度喘息性支气管炎效果

摘　　要：目的采用临床症状评分的方法,评价吸入型糖皮质激素（ICS）治疗婴幼儿轻度喘息性支气管炎的临床效果。方法 2015年1-2月,于青岛大学附属医院儿科选择临床诊断为轻度喘息性支气管炎婴幼儿40例,按1∶1比例随机分为对照组（常规治疗）和雾化组（常规治疗＋雾化吸入布地奈德治疗）,比较两组治疗前及治疗后24、48、72h的临床症状评分。结果对照组治疗后24h临床症状无明显改善（P〉0.05）,治疗后48、72h与前1个24h相比,临床症状明显改善（z=4.03、4.12,P〈0.05）;雾化组治疗后24、48、72h与前1个24h相比,临床症状均明显改善（z=3.97~4.13,P〈0.05）。治疗后24、48、72h时雾化组临床症状评分较对照组明显降低（t=6.99~17.61,P〈0.05）。雾化组治疗时间较对照组显著缩短（t=3.21,P〈0.05）。结论 ICS治疗婴幼儿轻度喘息性支气管炎能明显改善临床症状,缩短治疗时间。Objective Employing Clinical Symptom Score method,to assess the effectiveness of inhaled glucocorticoid treatment in babies with mild asthmatic bronchitis（MAB）. Methods Forty babies were selected for this study from those who were diagnosed as suffering from MAB-from January to February of 2015-in Department of Pediatrics of our hospital,and evenly randomized to control group（conventional treatment）and atomization group（conventional treatment plus Budesonide inhalation）.Clinical symptom scores were compared between the two groups-before and 24,48 and 72 hours after treatment. Results At 24 hours after the treatment,clinical symptoms of sick kids in the control group were not obviously improved（P〉0.05）,and after 48 and 72 hours,the symptoms notably improved（Z=4.03,4.12;P〈0.05）.In the atomization group,the symptoms were significantly improved at 24,48,and 72 hours after treatment（z=3.97-4.13,P〈0.05）,the clinical symptom scores being much lower than that in the control group（t=6.99-17.61,P〈0.05）.The time of therapy was shorter in the atomization group versus the control（t=3.21,P〈0.05）. Conclusion Inhaled corticosteroid therapy can significantly improve the clinical symptoms of babies with mild asthmatic bronchitis and shorten the treatment time.
