摘  要:目的探讨磁共振检查(MRI)对腰椎骨挫伤的诊断价值。方法对25例经临床治疗和观察证实的腰椎椎体骨挫伤病人的MRI资料进行回顾分析。结果本组25例病人共有48个腰椎椎体骨挫伤病灶,MRI T_1WI均表现为不规则斑片状、地图状及条状低信号,T_2WI表现为不规则高信号,脂肪抑制表现为明显高信号。其中累及L_1椎体2例,L_2椎体4例,L_3椎体6例,L_4椎体6例,L_5椎体7例。异常信号分布在椎体前1/3者8例,椎体中1/3者12例,椎体后1/3者21例,整个椎体累及者7例。结论 MRI对骨髓变化具有很高敏感度,能清楚显示腰椎骨挫伤的病变特点及严重程度,是诊断腰椎骨挫伤的首选方法。Objective To assess the value of MRI in the diagnosis of lumbar-vertebrae contusion(LVC). Methods A retrospective review was conducted for MRI data of 25 cases of LVC that confirmed by clinical treatment and observation. Results A total of 48 contusion lesions were found in the 25 cases.T_1WI of MRI showed irregular patchy,map like and strip low signal,and T_2WI showed irregular high signal on fat suppression.Of which,L_1 was involved in two cases,L_2 in four,L_3 in six,L_4 in six and L_5 in seven.Abnormal signal distribution was noted on 1/3 of anterior vertebral body in eight cases,on 1/3 of middle vertebral body in 12 cases,on 1/3 posterior vertebral body in 21 cases and the whole vertebral body was involved in seven cases. Conclusion MRI is highly sensitive to changes in bone marrow,which can clearly demonstrate the features of contusion of lumbar vertebrae and its severity of injury.The method is regarded be the first choice for the diagnosis of this kind of trauma.
