雷公藤多苷联合肾康丸治疗晚期糖尿病肾病的疗效观察

摘　　要：目的:研究雷公藤多苷联合肾康丸治疗晚期糖尿病肾病的临床疗效。方法:选取符合纳入标准的68例晚期糖尿病肾病患者随机分为对照组34例和治疗组34例,对照组每日口服3次雷公藤多苷,每次20 mg;治疗组在每日口服3次雷公藤多苷,每次20 mg的基础上再每日口服3次肾康丸,每次6 g,疗程3个月。治疗前以及治疗后分别检测患者的血白蛋白、肝功能、尿蛋白定量、血常规、肾功能以及血脂等指标。同时分别评价其临床疗效,副作用以及不良反应。结果:对照组患者治疗后显效10例,有效16例,无效8例;治疗组患者治疗后显效20例,有效12例,无效2例;两组无效人数比较有显著性差异,治疗组患者治疗后无效人数较对照组显著减少,具有显著性差异(χ2=7.505;P=0.023);治疗组治疗后尿蛋白含量较对照组低(t=-6.411,P<0.01),且两组患者治疗前后尿蛋白含量差值之间的比较也具有显著性差异,治疗组差异显著高于对照组差值(t=10.250,P<0.01);治疗组治疗前后血浆白蛋白、肌酐、尿素氮、总胆固醇和甘油三酯含量相比较具有显著性差异,而对照组在治疗前后相应数据没有显著性差异。治疗后两组患者血浆白蛋白、肌酐、尿素氮、总胆固醇和甘油三酯含量比较有显著性差异(t=4.283,P<0.01;t=-14.644,P<0.01;t=-7.004,P<0.01;t=-8.768,P<0.01;t=-5.334,P<0.01)。结论:雷公藤多苷联合肾康丸治疗晚期糖尿病肾病可以使蛋白尿明显减少,延缓病情发展。Objective：To research the curative effect of 68 cases with advanced diabetic nephropathy by using tripterygium wilfordii glycosides combined with Shenkang pills. Methods： 68 cases with advanced diabetic ne- phropathy were randomly divided into a control group and a treatment group. In the control group, we gave tripterygium wilfordii glycosides orally for three times a day, every time 20 mg;in the treatment group patients received tripterygium wilfordii and Shenkang pills, with the treatment for 3 months. Results：In the treatment group invalid number was significantly reduced and had a significant difference （X^2 = 7. 505 ; P = 0. 023 ） and the urinary protein in two groups after treatment was significantly different, and in the treatment group they were lower than those in the control group （t = -6. 411 ,P 〈 0.01 ）. Serum albumin, urea nitrogen, creati- nine, total cholesterol and three acyl glycerin in two groups after treatment had a significant difference （t = 4. 283,P 〈0.01 ;t = - 14. 644,P 〈0.01 ;t = -7. 004,P 〈0.01 ;t = -8.768,P 〈0.01 ;t = -5. 334,P 〈 0. 01 ）. Conclusion ： Tripterygium wilfordii glycosides combined with Shenkang pills in the treatment of ad- vanced diabetic nephropathy can reduce proteinuria significantly and delay the progression of the disease.
