腹腔镜输尿管切开取石术中结石移位因素分析及处理对策

摘　　要：目的探讨腹腔镜输尿管切开取石术中结石移位的因素和处理对策。方法选取2006年1月~2016年6月我院收治的403例输尿管中上段结石患者作为研究对象,回顾性分析其临床资料,总结腹腔镜输尿管切开取石术中结石移位的影响因素。结果 403例输尿管中上段结石患者中,22例出现结石移位,结石直径、肾积水程度、游离输尿管过程操作粗暴、结石段输尿管显露后是否采取相应防范措施是腹腔镜输尿管切开取石术中结石移位的危险因素。结论腹腔镜输尿管切开取石术中导致结石移位的因素较多,结石直径、肾积水程度、游离输尿管过程操作粗暴、结石段输尿管显露后是否采取相应防范措施是引起结石移位的危险因素,临床需加强对影响因素的预防,减少术中结石移位的发生,提高手术成功率。Objective To explore the factors of stone transferring and countermeasures in laparoscopic ureterolithotomy.Methods 403 patients with stones in the upper and middle parts of the ureter in our hospital from January 2006 to June 2016 were selected as research objects,their clinical data were retrospectively analyzed.The influence factors of stone transferring during laparoscopic operation in the patients with ureteral calculi were summarized.Results In 403 patients with stones in the upper and middle parts of the ureter,there were 22 patients occurred with stone transferring.The stone diameter,degree of hydronephrosis,rough operation of ureter dissection,and whether or not to take corresponding precautionary measures after exposure of ureter located with the stones were risk influence factors of stone transferring in laparoscopic ureterolithotomy.Conclusion There are many factors of stone transferring in laparoscopic ureterolithotomy including the stone diameter,degree of hydronephrosis,rough operation of ureter dissection,and whether or not to take corresponding precautionary measures after exposure of ureter located with the stones.In clinic,precaution should be strengthened on these influence factors aiming at reducing the occurrence of stone transferring during surgery and increasing operative success rate.
