剖宫产前后体位指导在提高剖宫产产妇母乳喂养成功率中的效果

摘　　要：目的研究并探讨剖宫产前后体位指导在提高剖宫产产妇的母乳喂养成功率中的效果。方法选取2013年1月~2015年12月我院产科收治的100例具有高危妊娠因素的选择性剖宫产产妇作为研究对象,采取随机数字表法将产妇分为对照组、观察组,每组50例。对照组实施常规护理,观察组在常规护理的基础上实施产前哺乳体位训练、产后喂养体位指导。比较产后第1天、第2天、第3天两组产妇的乳汁分泌量以及母乳喂养成功率,并对两组产妇的护理满意度进行比较。结果产后第1天,两组产妇乳汁分泌量以及母乳喂养成功率比较,差异无统计学意义（P〉0.05）;产后第2天、第3天,观察组产妇的乳汁分泌量以及母乳喂养成功率较对照组均明显更高（P〈0.05）。观察组产妇的护理满意度为98%,高于对照组的86%,差异有统计学意义（P〈0.05）。结论在剖宫产前后对产妇实施合理的哺乳体位指导,可有效促进产妇乳汁的分泌,提高剖宫产后婴儿的母乳喂养成功率。Objective To study and explore the effect of postural guidance before and after cesarean section in improving breastfeeding success rate of cesarean section maternal.Methods From January 2013 to December 2015,100 cases of pregnant women given selective cesarean section with high risk factors of pregnancy department of obstetrics in our hospital were selected as the research object and divided into control group and observation group by random number table,each group had 50 cases.The control group had received the routine nursing,the observation group had received prenatal breastfeeding posture training and postpartum feeding posture guidance on the basis of routine nursing.On the postpartum first days,second days,third days,the maternal milk secretion and the success rate of breast feeding were compared between two groups,and the nursing satisfaction was compared between two groups.Results On the postpartum first days,the maternal milk secretion and breastfeeding success rate were compared between two groups,and the difference was not statistically significant（P〉0.05）;On the postpartum second days,third days,the maternal milk secretion and breastfeeding success rate in the observation group were significantly higher than those in the control group（P〈0.05）.The nursing satisfaction of maternal in the observation group was 98%,compared with the control group of 86%,the difference was statistically significant（P〈0.05）.Conclusion Before and after the cesarean section,the implementation of prenatal breastfeeding posture guidance can effectively promote the maternal milk secretion,increase the success rate of caesarean baby breastfeeding.
