子宫圆韧带囊肿合并腹股沟斜疝彩色多普勒超声分析

摘　　要：目的通过对子宫圆韧带囊肿合并腹股沟斜疝的临床资料及彩色多普勒超声影像资料分析,以提高子宫圆韧带囊肿合并腹股沟斜疝的诊断率。方法回顾分析2013年1月至2016年5月,首都医科大学附属北京朝阳医院西院手术证实子宫圆韧带囊肿合并腹股沟斜疝18例患者的临床资料及声像图特点。结果 46例手术证实子宫圆韧带囊肿合并腹股沟斜疝中18例术前行超声检查,其中16例术前超声诊断子宫圆韧带囊肿合并腹股沟斜疝,1例诊断腹股沟斜疝漏诊圆韧带囊肿,1例误诊为股疝,诊断准确率88.9%(16/18)。子宫圆韧带囊肿超声表现为长椭圆形、梭形及不规则形,呈无或低回声,内部伴或不伴分隔例,CDFI分隔上可见血流信号。结论圆韧带囊肿和女性斜疝的发生有密切关系;高频彩色多普勒超声为圆韧带囊肿的诊断及鉴别诊断提供影像学依据;对圆韧带及圆韧带囊肿的认识有助于提高隐匿性腹股沟斜疝的诊断率。Objective To improve the diagnosis of hydrocele Nuck＇s canal with inguinal hernia, an analysis of clinical data and color Doppler ultrasound imaging data was made. Methods From January 2013 to May 2016, 18 cases hydrocele Nuck＇s canal with inguinal hernia which were confirm by operations in Bejing Chao-yang hospital were collected. Their clinical data and sonographic features were retrospectively analyzed. Results A total of 46 cases of hydrocele Nuck＇s canal with inguinal hernia were confirmed by surgery. Of these, 18 cases had undergone preoperative ultrasonography. There were 16 patients with a preoperative ultrasound diagnosis of both hydrocele Nuck＇s canal and inguinal hernia. One was failure to diagnosis with hydrocele Nuck＇s canal, and 1 case misdiagnosed as femoral hernia. The diagnostic accuracy was 88.9% （16/18）. Uhrasonography of hydrocele Nuck＇s canal showed long oval, spindle-shaped and irregular, no or low echo, with or without internal separation, and blood flow signal in CDFI. Conclusion Hydrocele Nuck＇s canal in women is closely related to the occurrence of inguinal hernia; color Doppler ultrasound imaging to provide a basis for hydrocele Nuck＇s canal diagnosis and differential diagnosis.
