摘 要:目的研究慈溪市人民医院呼吸内科2013—2015年细菌耐药率情况及抗菌药物用药频度(DDDs)间的关系,为临床合理用药提供选择依据。方法分析2013—2015年呼吸内科分离出的主要病原菌的耐药率及DDDs情况,采用SPSS.16.0统计软件分析其相关性。结果呼吸内科分离出的病原菌株数量最高的前5名为:嗜麦芽寡养单胞菌、鲍曼不动杆菌、阴沟肠杆菌、金黄色葡萄球菌,热带假丝酵母菌。DDDs值最高的前5名为左氧氟沙星、克拉霉素、头孢噻肟、哌拉西林他唑巴坦、制霉菌素。其中金黄色葡萄球菌对头孢噻肟的耐药率与其DDDs值存在相关性(r=0.99,P<0.05),其耐药率高于全院平均水平(P<0.05);阴沟肠杆菌对左氧氟沙星的耐药率与其DDDs值存在相关性(r=0.99,P<0.05),其耐药率与全院平均水平无显著性差异(P>0.05)。结论部分抗菌药物的DDDs值与细菌耐药率之间存在正相关,可作为临床选择抗生素治疗的依据之一。Objective To analyze the relationships between bacterial resistance and DDDs of antibacterial agents of respiratory medicine between 2013 and 2015 so as to provide reference for the rational clinical use of antibiotics. Methods The DDDs among inpatients and the drug-resistance rate of main clinical pathogens in respiration medicine from 2013 to 2015 were analyzed statistically. The data were processed simultaneously by SPSS 16. 0software. Results The top five bacteria were Eosinophilic malt oligotrophic,acinetobacter baumannii,sewer bacterium enterobacter,staphylococcus aureus and C. tropicalis. The top five DDDs involved levofloxacin,clarithromycin,cefodizime,piperacillin / tazobactam. The DDDs of cefodizime were positively correlated with the resistance of staphylococcus aureus to cefodizime( correlation coefficient,r = 0. 99),and the resistance was higher than the average level at the hospital( P 〈 0. 05). The DDDs of levofloxacin were positively correlated with the resistance of sewer bacterium enterobacter to levofloxacin( correlation coefficient,r = 0. 99),and the resistance was not singificantly different from the average level of the hospital( P 〉 0. 05). Conclusion The DDDs of some antibacterial agents are positively correlated with the rate of bacterial drug resistance,which can be used as a reference for clinical selection of antibiotics.
