摘　　要：目的分析西部地区20家医院门诊麻醉药品应用情况,考察其用药合理性。方法回顾性分析西部地区20家医院门诊麻醉药品配备品种和2015年1—5月麻醉处方数据,通过药物利用指数(DUI)、日均药费(DDC)排序法、用药金额与用药频度(DDDs)序号比,进行统计分析。结果 80%的医疗机构门诊配备麻醉药品小于9种。分层抽样法抽取麻醉处方共计679张,使用量最大的为缓释片,占总处方量的44.04%,其DDDs位居第1位。大多数缓释片和透皮贴剂的DUI大于1。速释片处方量和DDDs最少,价格低廉。大部分麻醉药品的用药金额与DDDs的序号比值均在1左右。结论麻醉药品门诊配备品种少,且供应不足;使用以缓释片占主导地位,基本趋于合理,但价格结构偏高。应强化麻醉药品规范化管理、临床合理应用和医疗人员的培训。Objective To evaluate the use of narcotic drugs by outpatient departments in 20 hospitals in western China and to facilitate rational use of narcotic drugs in clinic practice. Methods Data on narcotic drugs and prescriptions in outpatient departments of 20 hospitals in the west of China between January and May of 2015 were retrospectively analyzed. Drug utilization index（ DUI）,daily drug cost（ DDC）,and the ratio of the order of consumption sum to that of medication frequency（ DDDs） were used to evaluate the utilization of narcotic drugs. Results 80% of these outpatient departments had less than 9 types of narcotic drugs avaicable. According to the 679 narcotic prescriptions sampled by the stratified sampling method,the largest amount of narcotic drugs used was delayed-release tablets,which accounted for44. 04% of the total number of prescriptions. Also,their DDDs took the first place. Most delayed-release tablets and transdermal patches had a DUI greater than 1. Tablets had the smallest number of prescriptions and DDDs. The ratio of cost to DDDs for most of these narcotic drugs was close to 1. Conclusion The amount of marcotic drugs in outpatient departments is insufficient and in short supply. Delayed-release tablets have taken the dominant place,which are in rational use but are a little bit too expensive. It can be concluded that management of narcotic drugs,rational clinical application and medical personnel training should be enhanced and improved.
