摘　　要：目的:观察输卵管妊娠不同治疗方法对卵巢功能的影响。方法:随机将120例输卵管妊娠患者分为3组:A组药物治疗,B组行输卵管切除术,C组行输卵管保守手术,对比治疗后性激素水平变化,并采用阴道超声检测卵巢体积、基础窦卵泡数、子宫内膜厚度进行分析。结果:与A、C两组相比,B组卵泡刺激素FSH、黄体生成素LH水平均明显增加;雌二醇E2水平明显下降(P<0.05);A组、C组卵巢基础窦卵泡数及卵巢体积明显大于B组,子宫内膜厚度厚于B组(P<0.05);开腹手术组与腹腔镜手术组相比,FSH、LH水平均明显增加而E2水平明显下降,差异具有显著性(P<0.05);基础窦卵泡数、卵巢体积及子宫内膜厚度明显减少。(P<0.05)。结论:行输卵管切除术,明显影响患者卵巢的储备功能,对于有生育要求的患者,尽可能给予保守方法进行治疗;并且腹腔镜手术较开腹手术对患者卵巢功能的影响更小。Objective：To observe the effect of different treatment methods of tubal pregnancy on ovarian function.Methods：120patients with tubal pregnancy were divided into three group randomly：Group A treated with medical treatment;Group B treated with salpingectomy;Group C treated with tubal conservative surgery;Sex hormones were compared after treatment,and ovarian volume,number of basis antral follicles,and endometrial thickness measured by transvaginal ultrasound were analyzed.Results：Compared with group A,the FSH and LH levels of group B were significantly increased,while E2 level was significantly decreased（P〈0.05）;Ovarian based sinus follicles number and ovarian volume of group A and C were significantly higher than those of group B,and endometrial thickness of group A and C were thicker than that of group B（P〈0.05）;Laparotomy patients compared with laparoscopic surgery patients,FSH and LH levels were significantly increased and E2 level was significantly decreased,the difference was significant（P〈0.05）;Based antral follicle number,ovarian volume and endometrial thickness were significantly reduced（P〈0.05）.Conclusion：Tubal surgery significantly affects the function of ovarian reserve,so for patients with fertility requirements,they should be given the conservative treatment as far as possible;and the impact of laparoscopic surgery on ovarian function was smaller than open surgery on ovarian function.
