急性高容性血液稀释联合自体血回收回输技术对全髋关节置换手术血液保护效果及其安全性研究

摘　　要：目的研究分析急性高容性血液稀释联合自体血回收回输技术全髋关节置换手术血液保护效果及其安全性的影响。方法将本院行全髋关节置换手术治疗,预计出血量>600m L的125例患者分成两组,对照组55例患者行单纯AHH,研究组70例患者行AHH联合自体血回收回输技术,对比两组血液保护效果,术中自体血回输量及异体血输入量及并发症。结果两组T2、T3时间段Hb水平显著低于T1(P<0.01),但仍处于代偿范围,两组T1、T2、T3、T4时间段内MAP、HR、Sp O2、Pa O2、Hb、Hct、PT、PLT指标组内、组间对比(P>0.05);研究组自体血回输量多于对照组,异体血输入量少于对照组(P<0.01);两组寒战、荨麻疹等并发症对比(P>0.05)。结论急性高容性血液稀释联合自体血回收回输技术对全髋关节置换手术血液保护效果显著,且无严重并发症发生。Objective To study the effect of blood protection and safety in the treatment of acute and high volume blood dilution combined with autologous blood transfusion in total hip replacement surgery. Methods Total hip replacement surgery was performed in 125 patients,and 55 patients were treated with AHH,and the control group was treated with600 m L,and 70 patients were treated with AHH combined with autologous blood transfusion. Results The two groups of T2,T3 time Hb was significantly lower than that of T1（ P〈0. 01）,but is still in the scope of compensation,two groups of T1,T2,T3,T4,MAP,HR,time Sp O2,Pa O2,Hb,Hct,PT,PLT index group,the comparison between the two groups（ P〈0. 05 the study group）; Autologous blood transfusion amount more than the control group,the amount of blood transfusion was less than the control group（ P〈0. 01）; Two groups of chills,urticaria and other complications contrast（ P〈0. 05）. Conclusions Acute high volume blood dilution combined with autologous blood transfusion technology has a significant effect on the blood protection of total hip replacement surgery,and no serious complications occurred.
