急诊在C臂透视下闭合复位治疗儿童肱骨髁上骨折  

Emergency treatment of humerus supracondylar fracture in children by using closed reduction under C-shaped arm X-ray

摘　　要：目的：探讨急诊在C臂透视下闭合复位治疗儿童肱骨髁上骨折的手术治疗效果。方法对2007-2013年收治的94例儿童肱骨髁上骨折，均在C臂透视下行手法复位经皮克氏针交叉内固定治疗，并对临床资料进行回顾性分析。结果用Flynn标准评价，分析其手术方法及功能锻炼时效。结果94例4周后生成骨痂，3个月后取出内固定，无1例神经损伤。术前2例肱动脉损伤者，术中给予探查吻合；13例发生针道感染。于术后8周，12周，6个月判定功能，优良率97.9％。随访3~5年，无肘内外翻畸形。结论儿童肱骨髁上骨折，急诊在C臂透视下手法复位经皮克氏针内固定，可减少血管及神经损伤，进行早期功能锻炼，疗效满意。Objective To discuss the effect of emergency treatment humerus supracondylar fracture in childen by using closed reduction under C-shaped arm X-ray . Methods Humerous fracures in children through a retrospective analysis in our hospital from 2007 to 2013, and they were treated in 94 cases. Followed up from 6 months to 5 years, resulte were determined by Flynn standards, and analyzed the surgical methods and functional exercise. Results 94 cases in children were under C-arm X-ray technique reduction and percutaneous Kirschner wire fixction, callus created in 4 weeks, removed the fixation after 3 mouths, preoperative 2 cases of brachial artery injury, and do exploration anastomosis, 13 cases of pin tract infection, and in 8 weeks, 12 weeks, 6 mouths decision function, the excellent rate was 97.9%. 3 to 5 years of following-up, no elbow varus. Conclusion Emergency in C-arm X-ray technique closed reduction and percutaneous pinning for treating humerus supracondylar in children, can reduce the damage to blood vessels and nerves after treatment, can early exercise, can achieve satisfactory results.
