摘  要:目的验证新一代片段分析技术(advanced fragment analysis,AFA),对HCV基因分型及白细胞介素28B的2个SNP位点多态位点检测的可靠性,并探讨其在聚乙二醇干扰素联合利巴韦林抗HCV的疗效预判中的意义。方法收集132例慢性丙型肝炎患者外周血样本。同时随访1年~2.5年,并将患者分成2组,比较HCV不同基因型以及携带不同SNP位点患者在2组中的差异。结果 AFA对样本进行分型检测,与Sanger测序法比较,准确性达到100%,随访发现感染HCV亚型为1b的患者、或者rs12979860 CT型、或rs8099917 TG型患者干扰素治疗后,复发概率高。结论AFA法检测丙型肝炎病毒基因分型及rs12979860和rs8099917位点多态性,具有准确性好、通量高、操作简便等特点,适合于临床推广应用。Objective To investigate the reliability in the HCV genotyping and in the polymorphic loci detection on 2 SNP sites of IL- 28 B by advanced fragment analysis( AFA),and its significance in predicting the treatment efficiency of PEG- IFN combined with ribavirin for HCV. Methods Peripheral blood samples of 132 chronic hepatitis C patients were collected. All the patients were divided into 2 groups after 1- 2. 5 year's follow- up. The differences of genotype and patients carrying different IL- 28 B SNP sites in the 2 groups were compared. Results The HCV genotyping accuracy of AFA technology reached 100%compared with Sanger method. The patients infected with HCV 1b genotype had higher probability of recurrence. The patients with rs12979860 CT or rs 8099917 TG genotype were liable to relapse. Conclusion AFA technology with the advantages such as good accuracy,high flux,easy to operate and so on,and it can be applied in clinic. AFA method for the detection of hepatitis C virus genotyping and rs12979860 and rs8099917 polymorphism has good accuracy,high throughput,easy operation,which is suitable for the clinical application.
