核酸检测在HIV抗体不确定样本辅助诊断中的应用

摘　　要：目的研究核酸定量检测在人类免疫缺陷病毒(HIV)抗体检测不确定样本的辅助诊断中的应用。方法收集2014年-2015年宁波市HIV抗体检测结果不确定样本43例,随访获得患者WB抗体条带、病毒载量以及外周血CD4+T淋巴细胞计数的检测结果。研究在HIV抗体检测结果不确定时,以核酸定量检测结果作为判断依据进行辅助诊断的可行性。结果经随访,43例样本中38例病毒载量结果呈阳性,同时WB抗体条带发生进展,达到阳性判定标准。CD4+检测结果和流行病学资料显示其中8例为艾滋病患者,29例为HIV急性期感染者,1例缺少流行病学资料。5例病毒载量结果阴性,随访抗体条带转阴排除HIV感染。结论 WB不确定结果有检测方法和受检人群等客观原因造成。核酸定量试验在对感染早期、晚期患者等不确定样本的检测中显示出很好的敏感性和特异性,作为HIV检测辅助诊断的依据,具有实际意义。Objective To study the application of nucleic acid quantitative detection in the assistant diagnosis of HIV antibody indeterminate samples. Methods 43 HIV antibody indeterminate samples were collected in Ningbo HIV confirmation laboratory from 2014 to 2015. The detection of WB,viral load and CD4＋lymphocyte count were followed- up,to study the application of nucleic acid quantitative detection in the assistant diagnosis of indeterminate samples. Results During follow- up,the viral load results of 38 cases in 43 samples were positive,while WB antibody bands made process,reaching the positive standard.CD4＋detection results and epidemiological data showed 8 cases of AIDS patients and 29 cases of acute HIV infected patients,and that 1 case had no epidemiological data. viral load results in 5 cases were negative,followed- up antibody band negative exclused HIV infection. 1 case has no epidemiological data. The other 5 cases who have negative results in nucleic acid detection were excluded from infection with negative results in WB bands. Conclusion WB uncertain results are caused by objective reasons,such as the detection method and the subjects. Nucleic acid quantitative detection have showed great sensitivity and specificity in the detection of uncertain samples from the patients at the early and the advanced infection stages. As the assistant basis of HIV detection,it is of practical significance.
