蜈倍汤中药溻渍治疗慢性湿疹的临床疗效观察

摘　　要：目的:评价蜈倍汤中药溻渍治疗慢性湿疹的临床疗效。方法:纳入68例慢性湿疹患者,随机分为治疗组和对照组,每组34例。治疗组给予蜈倍汤中药溻渍治疗,对照组给予5%水杨酸软膏联合樟脑软膏外涂治疗,治疗周期为4周。评价两组患者的临床疗效,采用改良的EASI评分法,评价并比较两组患者的客观症状(包括皮损面积、皮损形态)和自觉症状(包括瘙痒、睡眠影响)的评分。结果:经治疗后,治疗组和对照组的临床总有效率分别为93.33%和74.19%,两组临床疗效比较,差异有统计学意义(P<0.05);治疗后,治疗组患者的皮损面积、红斑/丘疹、角化/脱屑、浸润/肥厚/苔癣样变、瘙痒、睡眠影响评分较治疗前均降低(P<0.05),对照组患者的红斑/丘疹、角化/脱屑、浸润/肥厚/苔癣样变、瘙痒、睡眠影响评分较治疗前亦降低(P<0.05),且治疗组患者的皮损面积、红斑/丘疹、睡眠影响评分低于对照组(P<0.05);治疗后,两组患者的症状总积分较治疗前均显著降低(P<0.05),且治疗后第3周、4周,治疗组患者的症状总积分明显低于对照组(P<0.05)。结论:蜈倍汤中药溻渍治疗慢性湿疹具有较佳的功效,能明显改善患者的临床症状。Objective ： To evaluate the clinical efficacy of fomentation with ＂Wubei Decoction＂ in the treatment of chronic eczema. Methods： Sixty-eight patients with chronic eczema were included and randomly divided into the treatment group and control group with 34 cases in each group. The patients in the treatment group were treated with fomentation of ＂ Wubei Decoction＂ and the patients in the control group were treated with external application of 5% salicylic acid ointment combined with camphor ointment, with a course of 4 weeks. The clinical effect was evaluated, and the scores of the objective symptoms including skin lesion area and morphology and the subjective symptoms including pruritus and sleep were assessed by modified EASI scores. Results： After treatment, the total effective rates of the treatment group and control group were 93.33% and 74.19% respectively, with statistical difference between the two groups （ P 〈 0.05 ）. After treatment, the scores of skin lesion area, erythema/papule, keratinization/desquamation, infihration/dermatauxe/lichenification, pruritus and sleep in the treatment group were decreased （ P 〈 0. 05 ）, the scores of erythema/papule, keratinization/desquamation, infiltration/ dermatauxe/lichenification, pruritus and sleep in the control group were also decreased （ P 〈 0.05 ）, and the scores of skin lesion area, erythema/papule and sleep in the treatment group were lower than those in the control group （P 〈 0.05 ）. After treatment, the total scores of symptoms in both groups were significantly decreased （ P 〈 0.05 ）, and three and four weeks after treatment, the total scores of symptoms in the treatment group were obviously lower than those in the control group （ P 〈 0.05 ）. Conclusion： The Chinese medical fomentation with ＂ Wubei Decoction＂ has better effect in treating patients with chronic eczema and can obviously improve the clinical symptoms.
