基于医保支付方式改革的医疗卫生资源配置均衡化研究——以镇江市为例

摘　　要：目的:以镇江市医保支付方式改革为切入点,探索支付方式对于医疗卫生资源配置均衡化的杠杆调节作用。方法:选取镇江市两家三甲医院和四家社区医院为研究对象,调查和分析"总额预算下按人头点数法"卫生资源配置的作用。结果:"总额预算"支付方式具有确保医保基金收支平衡、引导居民就医行为、促进卫生资源向基层倾斜等政策性效果。结论:制定合理、科学的医保支付方式确有促进医疗卫生资源均衡化的作用,同时应强化预算总额当期控制,注重预算绩效评价,加强基层医疗能力建设,促进医保基金向社区预算投入。Objective This paper takes Zhenjiang medical insurance payment reform as the breakthrough point to explore the e- qualization of payment for medical and health resources allocation lever adjustment. Methods This paper chooses two third class A hos- pitals of Zhenjiang and four community hospitals, investigation under the ＂total budget per head points method＂ the role of the alloca- tion of health resources. Results the total budget payment has to ensure that the medical insurance fund balance of payments, guide residents medical behavior, and promote health resources to grassroots tilting, the policy effect. Conclusion making rational and scien- tific health care payment does promote the medical and health resources equalization effect, at the same time we should strengthen budget total current control, pay attention to the budget performance evaluation; To strengthen the construction of ability of primary care, and promote the medical insurance fund to the community budget.
