CD64感染指数与PCT对COPD患者急性加重期细菌感染的临床诊疗研究

摘　　要：目的观察慢性阻塞性肺疾病急性加重期(AECOPD)患者细菌感染时中性粒细胞CD64感染指数和降钙素原(PCT)的水平变化,探讨CD64感染指数和PCT检测在AECOPD细菌感染中的诊疗价值。方法选择2013年10月-2015年10月医院收治132例AECOPD患者,分析其医院感染率、感染病原菌分布,对比患者治疗前后CD64感染指数和PCT的变化情况。结果 132例AECOPD患者发生医院感染76例,感染率为57.58%;感染患者中培养出病原菌76株,其中革兰阴性杆菌54株占71.05%,革兰阳性球菌22株占28.95%;细菌感染患者经过治疗后,CD64感染指数和PCT水平均显著低于治疗前,治疗前与治疗后对比差异有统计学意义(P<0.05)。结论血清CD64和PCT检测是判断AECOPD患者是否细菌感染的敏感指标,为AECOPD细菌感染的诊断和鉴别提供依据,同时也为患者的治疗和预后评估提供了临床指导。OBJECTIVE To investigate the changes of CD64 neutrophil infection index and procalcitonin（PCT）levels in patients with chronic obstructive pulmonary disease and acute exacerbation（AECOPD）cells infection,and the clinical values of CD64 infection index and PCT detection in AECOPD bacterial infection.METHODS A total of 132 cases of AECOPD patients in the hospital from Oct.2013 to Oct.2015 were selected.The hospital infection rate,infection pathogen distribution,changes of CD64 infection index and PCT before and after the treatment of patients were analyzed.RESULTS Totally 76 cases of patients occurred nosocomial infections in 132 cases of AECOPD patients,and the infection rate was 57.58%.Totally 76 strains of pathogens were isolated,including54 strains of gram-negative bacteria,accounting for 71.05%,and 22 strains of gram-positive bacteria,accounting for 28.95%.The CD64 infection index and PCT level of patients after treatment were significantly lower than those of before treatment（P〈0.05）.CONCLUSION Serum CD64 and PCT detection is sensitive indicator to determine whether a bacterial infection in patients with AECOPD,and it provides not only the basis for diagnosis and identification of AECOPD bacterial infection,but also clinical guidance for the treatment and prognosis of patients.
