泌尿结石感染患者血清降钙素原与C-反应蛋白及白细胞介素6表达的诊断意义

摘　　要：目的探究血清降钙素原(PCT)、C-反应蛋白(CRP)及白细胞介素6(IL-6)对泌尿结石感染的联合诊断意义,为患者的临床治疗提供参考。方法选择2011年2月-2014年12月在医院治疗的泌尿结石合并感染患者46例为观察组,另选取46例正常人群为对照组,比较两组受试者血清PCT、CRP及IL-6各项指标情况,分析其对于泌尿结石合并感染患者的诊断意义。结果对照组CRP、PCT及IL-6检测结果分别为(2.3±1.2)mg/L、(0.29±0.1)ng/L及(3.4±4.2)ng/L,而观察组患者分别为(17.5±2.4)mg/L、(5.7±1.8)ng/L及(76.4±25.6)ng/L,两组各项指标进行比较差异有统计学意义(P<0.05);观察组患者CRP检测的特异性为84.8%,敏感性为76.1%,诊断符合率为82.6%,PCT检测的特异性为89.1%,敏感性为82.6%,诊断符合率为89.1%,IL-6检测的特异性为86.9%,敏感性为76.1%,诊断符合率为86.9%,PCT检测的敏感性以及特异性较高,三项指标联合检查可提高患者确诊率。结论血清PCT、CRP及IL-6可作为泌尿结石合并感染患者的诊断指标,具有较高的临床意义。OBJECTIVE To investigate the significance of serum procalcitonin（PCT）,C-reactive protein（CRP）and interleukin-6（IL-6）in the joint diagnosis of infection of urinary calculi,so as to provide references for clinical treatment of patients.METHODS A total of 46 cases urinary stones coinfected patients in our hospital from Feb.2011 to Dec.2014 were selected as observation group,and another 46 cases of normal people were selected as control group.The serum PCT,CRP,and IL-6indexes of the two groups were compared,and its implications for the diagnosis of infection in patients with urinary stones were analyzed.RESULTS The CRP,PCT,and IL-6of control group were（2.3±1.2）mg/L,（0.29±0.1）ng/L,and（3.4±4.2）ng/L,while in observation group were（17.5±2.4）mg/L,（5.7±1.8）ng/L,（76.4±25.6）ng/L,and the differences were significant（P〈0.05）.In observation group,the specific detection of CRP was 84.8%,the sensitivity was 76.1%,and the coincidence rate of diagnosis was 82.6%;specificity of PCT detection was 89.1%,the sensitivity was 82.6%,and the coincidence rate of diagnosis was 89.1%;the specificity of IL-6detection was 86.9%,the sensitivity was 76.1%,and the diagnostic coincidence rate was 86.9%.The sensitivity and specificity of PCT detection were the highest,and the joint detection of the three indexes could improve the diagnosis rate.CONCLUSIONSerum procalcitonin,C-eactive protein and interleukin-6can be used as diagnostic indicators of urinary stones coinfected patients with high clinical significance.
