万古霉素缓释系统与Wright硫酸钙对骨折内固定术后骨感染治愈率的分析

摘　　要：目的探讨万古霉素缓释系统与Wright硫酸钙治疗创伤后骨折内固定术后骨感染的治疗效果,为创伤后骨折抗感染治疗提供依据。方法对2012年1月-2015年8月医院收治的创伤骨折内固定术后骨感染患者67例进行回顾性分析,其中应用万古霉素缓释系统与Wright硫酸钙的35例患者为研究组,敏感抗菌药物静脉注射的32例患者为对照组,比较两组患者感染清除率、骨折愈合率、骨折愈合时间和再手术率等。结果研究组患者Ⅰ、Ⅱ期感染清除率和愈合率分别为80.00%,94.29%和94.29%显著高于对照组患者50.00%,75.00%和75.00%,再次手术率研究组为20.00%显著低于对照组50.00%,差异有统计学意义(P<0.05);研究组患者骨折和骨腔愈合时间为(4.1±0.4)d和(5.2±0.5)d,显著短于对照组(5.3±0.7)d和(6.8±0.8)d,差异有统计学意义(P<0.05);两组患者病灶部位病原菌均以金黄色葡萄球菌为主,两组病原菌构成比较,差异无统计学意义。结论万古霉素缓释系统与Wright硫酸钙治疗骨折内固定术后骨感染,感染清除率和愈合率均较高,值得临床推荐应用。OBJECTIVE To investigate the effect of the treatment of bone infection after trauma with the vancomycin-release system and Wright calcium sulfate,so as to provide the basis for the treatment of post traumatic and fracture resistance.METHODS A retrospective analysis was made on 67 cases of postoperative patients with bone infection in our hospital from Jan.2012 to Aug.2015,including 35 patients with vancomycin-release system and Wright calcium sulfate as study group,and 32 cases with sensitive antibiotics as control group.The infection clearance rate,fracture healing rate,healing time of fracture,and reoperation rate were compared between the two groups.RESULTS In study group,the clearance rate and healing rate of I infection,II infection were 80.00%,94.29% and 94.29%,which were significantly higher than 50.00%,75.00% and 75.00% of control group,and the reoperation rate was 20.00%,which was significantly lower than 50.00% of control group（P〈0.05）.The fracture healing time and bone healing time of study group were（4.1±0.4）d and（5.2±0.5）d,which were significantly shorter than（5.3±0.7）d and（6.8±0.8）d of control group（P〈0.05）.The lesions of the disease of the two groups of patients were mainly Staphylococcus aureus.CONCLUSION The cure rate and the healing rate of the treatment of the fracture internal fixation with the antibiotic delivery system are high,and it is worthy of clinical application.
