摘　　要：目的了解克山病病区居民的异常心电情况,为病情评估提供量化指标及依据。方法采用非概率抽样的方法,选取全国共13个省120个病区县,对病区居民进行心电图描记,克山病的诊断依据《克山病诊断》(WS/T210-2011)。结果共描记42 489份心电图,异常心电图6 900份,总检出率为16.2%,男性异常检出率为14.0%,女性异常检出率为16.9%,性别间有统计学意义(P<0.05)。异常心电图指数评分以1-3分为主,病区总评分为1.06±0.246,克山病患者的总评分为1.08±0.282。8项异常心电改变以T波和(或)ST段改变、束支传导阻滞居多。心功能分级以Ⅰ级为主,但慢型克山病主要表现为Ⅱ级。结论本研究提出的异常心电图指数可作为病情严重程度的量化指标,比较病人个体及病区群体之间的异常心电程度。Objective To master the situation of abnormal ECG of the residents in Keshan Disease endemic areas, and provide an quantitative indicators and basis for the assessment of the disease. Methods We selected 120 townships of 13 provinces based on non-probability sampling. Based on Standard of Diagnosis of KD （WS/T210-2011）, residents were surveyed by electrocardiograms （ECG）. Results A total of 6 900 cases of abnormal electrocardiograms were detected in 42 489 electrocardiograms, and the abnormal rate of electrocardiogram is 16.2%, respectively. The comparison of the rates between male （ 14.0% ） and female （16.9%） reaches statistical significance （ P 〈0.05）. The abnormal ECG index score was 1-3, the average score of residents in Keshan Disease endemic areas is 1.06±0. 246 and 1.08±0.282 for KD patients. The changes of ST segment and （or） T waves and bundle branch block are the main forms of expression of the 8 abnormal electrocardiogram. According to the NYHA functional grading method, the largely are grading Ⅰ , while the patients of chronic KD are grading Ⅱ. Conclusion The abnormal ECG in- dex which proposed in this survey can be used as a quantitative indicator for comparison between individuals and groups.
