2015年吉林省克山病病情监测结果分析

摘　　要：目的掌握吉林省克山病患病及发病趋势,为防治克山病提供科学依据。方法选取克山病病区为监测点,按照《全国克山病监测方案》要求,对监测点常住人口进行病例搜索和病区监测。结果 5年累计搜索医疗机构总个数11 134个次,共搜索出心肌疾病873例,疑似克山病364例,慢型克山病264人,其中新确诊慢型克山病1例。共检诊15931人,检出克山病61例,总检出率3.8‰,其中慢型6例,检出率为0.4‰;潜在型55例,检出率为3.5‰,未发现急型和亚急型病人。全省心电图检查15 931人,出现异常心电图4 782人,异常率为30%。综合潜、慢克患病率情况,此次19个县全部达到控制标准,其中11个县达到了消除标准。结论各病区县病情差异较大,仍有新病例检出,克山病仍然是一个严重的地方性公共卫生问题,应加强克山病监测工作,控制和减少克山病的发生和发展。Objective To investigate the tendency of prevalence and incidence of Keshan disease （KD） in Jilin province, and to provide scientific basis for the control and prevention of KD. Methods Based on the national KD surveillance programme requirements, the residents in 19 surveillance sites were surveyed. Results In the five years, the medical institutions were searched 11 134 times, 873 records of cardiomyopathics, 364 suspected KD cases and 264 chronic KD cases including 1 newly diagnosed were found at the surveillance sites. 15 931 residents were investigated and 61 KD cases were confirmed, with a total incidence rate of 3.8%0. Among the 61 KD cases, 6 cases were chronic（0.4%0） and 55 cases were latent（3.5%0）. No acute or subacute cases were found. Of the 15 931 residents receiving ECG examination, 4 782 showed abnormal ECG （30%）. According to the prevalence situation, all the 19 counties reached control standard, and 11 of them reached elimination standard. Conclusion The results showed that there were significant differences among the disease areas. New KD cases were still found. KD is still one of the serious endemic public health issues. Surveillance should be emphasized to control and reduce the occurrence and development of KD.
