断层超声评估肛提肌肌力的可行性探讨

摘　　要：目的探讨盆底超声测量肛提肌尿道间隙（LUG）评估肛提肌肌力的可行性。方法对2014年9月~2016年6月在江苏省徐州市妇幼保健院产后门诊就诊的75例产后女性进行盆底肌力测定及会阴盆底三维超声检查,分别采集静息期及收缩期的肛提肌裂孔图像,在断层超声图像上测量左右侧LUG。根据盆底肌力测定结果,分成肌力正常组（n=60）和肌力降低组（n=15）,比较不同阶段（Ⅱ类肌纤维工作状态、Ⅰ类肌纤维工作状态、Ⅰ类和Ⅱ类肌纤维混合工作状态）两组间LUG值的差别。结果静息及缩肛期盆底Ⅱ类肌纤维、Ⅰ类肌纤维及Ⅰ类和Ⅱ类肌纤维混合工作状态下肌力降低组LUG值均明显高于肌力正常组,差异均有统计学意义（均P〈0.05）。结论经会阴盆底三维超声测量LUG可有效评估产后女性盆底收缩功能,有较好的临床应用前景。Objective To explore the feasibility of evaluation of levator ani muscle strength by measuring levator-urethra gap（LUG） with pelvic ultrasound. Methods In this study, transperineal 3D pelvic ultrasound and pelvic floor muscle strength testing were performed on 75 postpartum women who visited the Postpartum Clinic in Xuzhou Maternity and Child Health Care Hospital from September 2014 to June 2016. Those women＇s images of levator hiatus at rest and during contraction were acquired, and the left LUG and the right LUG were measured on tomographic ultrasound imaging. According to the results of pelvic floor muscle strength testing, the research objects were divided into the normal strength group（n=60） and the decreasing strength group（n=15）. The differences of LUG between the two groups at different stages（working states of type Ⅱ muscle fibers, type Ⅰ muscle fibers and the mixed working state of type Ⅰ and type Ⅱ muscle fibers） were compared. Results At rest and during contraction, the LUG values of decreasing strength group of type Ⅱ muscle fibers, type Ⅰ muscle fibers and mixed working state of type Ⅰ and type Ⅱ muscle fibers were higher than those of the normal group, with statistically significant differences（P〈0.05）. Conclusion To measure the LUG with transperineal 3D pelvic ultrasound appears to be valid and reliable for evaluation of levator strength, and has prosperous clinical application prospects.
