锁骨钩钢板治疗锁骨远端骨折和肩锁关节脱位的临床效果分析

摘　　要：目的探讨锁骨钩钢板治疗锁骨远端骨折和肩锁关节脱位的效果和安全性。方法同期收治的96例患者中NeerⅡ型锁骨远端骨折患者54例,TossyⅢ型肩锁关节脱位患者42例,根据患者病情及手术特征,采用随机数字表法分为锁骨钩钢板治疗组和克氏针张力带治疗组各48例。两组患者分别采用锁骨钩钢板内固定治疗和克氏针张力带钢丝、螺钉固定治疗,比较两组患者的手术时间、术中出血量、伤口愈合时间、住院天数,并随访观察两组患者的预后及并发症情况。结果两组患者的手术时间、术中出血量差异无统计学意义(P>0.05);锁骨钩钢板治疗组患者的住院天数和伤口愈合时间均明显短于克氏针张力治疗组,差异有统计学意义(P<0.05);锁骨钩钢板治疗组患者的肩关节功能改善优良率和内固定效果优良率均明显高于克氏针张力带治疗组,差异有统计学意义(P<0.05);同时,锁骨钩钢板治疗组患者的并发症发生率也明显低于克氏针张力带治疗组,差异有统计学意义(P<0.05)。结论锁骨钩钢板对锁骨远端骨折和肩锁关节脱位有较好的治疗效果,且并发症发生率少,可在临床上推广应用。Objective To discuss the effect and safety of Clavicular hook plate in the treatment of distal clavicle fracture and dislocation of the shoulder joint. Methods 54 patients with Neer Ⅱ distal clavicle fractures and 42 patients with Tossy Ⅲ acromioclavicular joint dislocation were randomly divided into two groups. Patients of two groups were treated by clavicular hook plate and Kirschner wire, respectively. The operation time, intraoperative blood loss, wound healing time and hospitalization days, prognosis and complications of patients in two groups were compared. Results There were no statistically significant differences in operation time and intraoperative blood loss of the two groups（P〉 0.05）. The wound healing time and hospitalization days of the Clavicular hook plate group were shorter than that of Kirscbner wire group （P〈0.05）. The rate of good effect of shoulder joint function improving and effect of internal fixation of the two groups were high. The incidence rate of complications of Clavicular hook plate group was obviously lower than that of the Kirschner wire group （P〈0.05）. Conclusion Clavicular hook plate can get good effect in the treatment of distal clavicle fracture and dislocation of the shoulder joint.
