摘 要:目的:探讨H型高血压患者白细胞内源性硫化氢(H2S)产生速度的影响因素。方法 :选取2012年10月~2013年12月在解放军总医院国际医学中心进行查体的高血压患者189例作为研究对象,根据同型半胱氨酸(Hcy)水平进行分组分析。同时设定对照组90例。所有入选对象测定Hcy水平及白细胞H2S产生速率,分析各组白细胞内H2S产生速率的差异,采用相关及回归分析探讨可能影响其变化的因素。结果 :H型高血压与非H型高血压患者白细胞H2S产生速率无统计学差异(P〉0.05);血压控制不佳者白细胞H2S的生成速度(nmol/μg)显著低于Hcy水平正常者(0.187±0.094 vs 0.229±0.139,P〈0.05),在血压控制组的患者中同样表现出这种趋势,但无统计学差异(P〉0.05)。血压控制未达标的患者中,白细胞H2S产生速率与平均动脉压明显负相关(P〈0.01),与Hcy负相关(P〈0.05)。白细胞H_2S产生速率与血浆IL-10水平显著相关(P〈0.01)。结论 :H型高血压患者白细胞内H2S产生速度与血压水平有关,控制不佳的患者其产生速度受Hcy水平的影响,升高的Hcy水平伴随较低内源性H2S生成速度;炎症因子可能是两者作用的交叉点之一。Objective:To investigate the generation characteristic of leukocyte-derived hydrogen sulfide of hypertensive person with high level homocysteine.Methods:One hundred and eighty-nine cases of hypertensive and 90 control cases were selected,all these persons came from PLA General Hospital from Oct 2012 to Dec2013.The hypertensive cases were divided into two groups according to the level of homocysteine.The level of plasma homocysteine and the production rate of hydrogen sulfide in leukocytes were measured in all subjects.In addition,the levels of IL-6,IL-10 and TNF-α were measured.The relationship of the leukocyte-derived hydrogen sulfide production rate,plasma cytokines and the blood pressure was analyzed.Results:There was no difference in the leukocyte production rate of hydrogen sulfide(nmol/μg/h)between normal homocysteine hypertensive and H type hypertension group(0.209±0.081 vs 0.207±0.090,P〉0.05).In those uncontrolled hypertensive subjects the production rate of hydrogen sulfide in leukocytes was significantly lower in H type hypertensive than normal homocysteine hypertensive group(0.187±0.094 vs 0.229±0.139,P〈0.05).In those uncontrolled hypertensive subjects,there was a negative relationship between leukocyte production rate of hydrogen sulfide and blood pressure(P〈0.01),homocysteine level(P〈0.05)and IL-10(P〈0.01).Conclusion:The production rate of the leukocyte-derived hydrogen sulfide is related with blood pressure in H type hypertension.
