简化McGill表评定“面痛三针”结合中药对PTN的影响

摘　　要：目的用简化McGill量化表评定"面痛三针"结合中药对PTN的影响。方法 75例原发性三叉神经痛患者随机分成中药组、针刺组、针药组,每小组25例。中药组辨证选方、针刺组"面痛三针"加循经取穴、针药组前两种治法均用。每周6次,1周为一疗程,观察并收集治疗4周前后患者痛阈值和简化McGill量化表中PRI感觉项、PRI情感项、PRI总分、VAS以及PPI项评分。结果三组在治疗后痛阈值较治疗前明显提高,而量化表中PRI感觉项、PRI情感项、PRI总分、VAS以及PPI评分均较治疗前降低明显,具有显著差异(P<0.05)。治疗后针药组痛阈值较中药组、针刺组升高最显著,具有显著差异(P<0.05);针药组量化表中PRI感觉项、PRI情感项、PRI总分、VAS以及PPI评分降低最明显,具有显著差异(P<0.05)。结论三组中,"面痛三针"结合中药治疗减轻PTN患者的疼痛程度最显著,并可改善患者的不良情绪,其效果优于单纯的中药或针刺治疗。Objective To estimate the clinical effect of the combination of ＂face pain three needles＂ and traditional Chinese medicine on PTN with simplified McGill pain questionnaire. Methods 75 eases of patients suffered from primary trigeminal neuralgia were randomized into traditional Chinese medicine group, acupuncture group and needle medicine group, with 25 in each group. The doctors used syndrome differentiation choice for the traditional Chinese medicine group. Acupuncture group used ＂ pain three needles＂ and aeupoint along the meridian. Needle medicine group applied the methods of traditional Chinese medicine group and acupuncture group. All the patients received treatment six times a week, and we regarded six times as one course, with four courses in all. Simplified McGill pain questionnaire and the pain threshold were adopted to assess the clinical effect of the three groups before and after the treatment. The pain threshold and PHI sensory items, PRI emotional items, PRI total score, VAS and PPI score in the simplify the MeGill quantization table in the 4 weeks before and after the treatment were observed and collected. Results After four weeks in the three groups, the pain threshold increased, but PRI-sensory seore, PRI-affective score, total score, VAS and PPI scores decreased obviously, and there were highly significant differences （ P 〈 0. 05）. After treatment, the pain threshold in the needle medicine group increased the most obviously than the other groups, and there were statistically significant differences （P 〈 0.05）. But PRI-sensory score, PRI-affeetive score, total score, VAS and PPI scores in the needle medicine group decreased the most obviously than the other groups, and there were statistically significant differences （ P 〈 0. 05 ）. Conclusion In the three groups, the ＂face pain three needle＂ combined with traditional Chinese medicine treatment can effectively reduce face pain and psychologic status of patients who suffered from PTN. It is the best treatment for PTN.
