针刺疗法治疗痴呆的临床研究进展

摘　　要：目的分析总结近10年来有关针刺疗法治疗痴呆的临床研究文献,明确针刺疗法治疗痴呆的临床研究进展。方法分别以针刺、针刺治疗及痴呆为检索词,2006—2015年为检索条件,对中国知识资源总库——CNKI系列数据库进行检索,并对检索结果进行分析整理。结果共筛选出32篇相关文献,发现研究主要以治疗血管性痴呆为主,对其他分型的临床实验研究报道较少。在研究方法上,大多数为有系统的随机对照试验设计(RCT),亦见有设置随访;其余则为单纯的临床对照试验(CCT)和病例报告;但多为疗效观察研究,有关痴呆的针刺机理研究报导尚不多见;无统一的诊断与疗效评定标准,无法客观评估临床疗效,机理探讨也欠深入。结论针刺疗法治疗痴呆具有操作简单方便,毒副作用小的优势,疗效满意,今后应进一步完善针刺疗法治疗痴呆的临床研究实验设计及分型研究,以阐明针刺疗法治疗痴呆的作用机理,使之更好地应用于临床。Objective To analyze the clinical research progress of acupuncture therapy in treating dementia in recent 10 years. Methods With acupuncture, acupuncture therapy and dementia as the search terms, from 2006 to 2015 as a search condition, China National Knowledge Infrastructure （CNKI） was retrieved, and the retrieval results were collected and analyzed. Results It is found that most of the literatures were studying the effect of acupuncture treatment on the vascular deraentia but less on the other subtypes in all 32 articles. For the researching strategy, systematic randomized control trial （RCT） with follow-up observation and the rest of them related to clinical control trial （CCT） and case reports were performed, most of the literatures were demonstrating on the effect of acupuncture treatment on dementia but less on the treating mechanism. Meanwhile, the diagnostic criteria and curative effect assessment were not aligned which hindered the objective evaluation of curative effect and treating mechanism of acupuncture therapy on dementia. Conclusion The review found promising evidence for the use of acupuncture in the treatment of dementia on improving the related symdrome. However, the results were limited by methodological flaws and the subtypes limitation. To confirm the therapeutic effect of various kinds of acupuncture therapy, the experimental design should be improved for the clinical research on the therapies and other subtypes＇ study, so that the therapeutic effect and treating mechanism of the therapies can be further testified and better applied to the ae.upuncture practice on dementia.
