三味清热止痒洗剂联合糖皮质激素治疗特应性皮炎疗效观察

摘　　要：[目的]探讨三味清热止痒洗剂联合糖皮质激素治疗特应性皮炎的临床疗效。[方法]随机数字表格法将符合纳入标准的64例特应性皮炎患者分为两组，各32例，对照组采取糖皮质激素治疗，观察组在对照组基础上联合三味清热止痒洗剂治疗，均干预4周，疗程结束后比较两组临床疗效、不良反应，并评价两组治疗前后特应性皮炎评分指数（SCORAD）及Th1／Th2细胞因子指标（IL-2、TNF-α、IFN-γ、IL-4、IL-6、IL-10）。[结果]观察组治疗总有效率93．75％显著高于对照组的71．87％（P〈0．05）；两组治疗后SCORAD总分、IL-4、IL-6、IL-10水平较治疗前均明显下降，IL-2、TNF-α、IFN-γ水平较治疗前均明显上升，差异有统计学意义（P〈0．05）；观察组治疗后SCORAD总分、IFN-γ、IL-4、IL-6水平与对照组同期比较差异有统计学意义（P〈0．05）；两组患者治疗期间均无明显不良反应发生。[结论]三味清热止痒洗剂联合糖皮质激素能明显改善特应性皮炎患者临床症状，安全有效，可能与三味清热止痒洗剂对Th1／Th2细胞因子有效调节有关。[Objective]To investigate the clinical effects of Sanwei Qingre Zhiyang Lotion combined with glucocorticoids in treatment of atopic dermatitis.[Methods]Sixty-four patients with atopic dermatitis who met the inclusion criteria were randomized into two groups , 32 cases in each . The control group was treated with glucocorticoids. The observation group was treated with Sanwei Qingre Zhiyang Lotion on the base of the former. Both of the two groups had been intervened for a period of 4 weeks. By the end of treatment, the clinical curative effects and adverse reactions were compared between the two groups. The atopic dermatitis score index （SCORAD） and Th1/Th2 cytokine indexes （IL-2, TNF-α, IFN-γ, IL-4, IL-6, IL-10） in the two groups were evaluated before and after treatment.[Results]The total effective rate in the observation group （93.75 %） was significantly higher than that in the control group （71.87%） （ P 〈0.05）. After treatment, the total scores of SCORAD, levels of IL-4, IL-6 and IL-10 in the two groups were significantly lower than those before treatment, while levels of IL-2, TNF-α and IFN-γ were significantly higher than those before treatment （ P 〈0.05）, the difference was statistically significant （P 〈0.05）; After treatment, the SCORAD total score, IFN-γ, IL-4 and IL-6 levels in the observation group were compared with those of the control group at the same period of time, the difference was statistically significant （ P 〈0.05）.[Conclusion]Sanwei Qingre Zhiyang Lotion combined with glucocorticoids can obviously relieve the clinical symptoms of patients with atopic dermatitis with safety and effectiveness. This may be related to the effective regulation of Th1/Th2 cytokines in the Sanwei Qingre Zhiyang Lotion.
