摘 要:诱导心肌细胞修复和再生的干细胞治疗技术已成为治疗心力衰竭(心衰)的一种新方法,将有望彻底改变心衰的治疗现状。干细胞治疗缺血性心衰能持续增加活力组织和局部收缩期室壁厚度从而减少心肌瘢痕的形成;对非缺血性心衰的治疗能改善心肌收缩力、功能活动和激活神经内分泌机制。目前关于干细胞移植治疗心衰的研究结果尚未达成一致,考虑心衰患者的复杂性,很难定义一种干细胞治疗适合所有的患者,而未来的干细胞治疗策略应该注重个体化治疗,对患者不同发病阶段能应用最佳的细胞类型、治疗剂量、注射途径和治疗频次。Currently, cell-based technology is emerging as a novel therapy with the potential to dramatically transform the treatment of heart failure through inducing myocardial repair and regeneration. The therapy of stem cells were shown to reduce myocardial scar burden with concomitant increase in viable tissue and regional systolic wall thickness in ischemic heart failure. Transplantation of stem cells can improve myocardial performance, functional capacity and neurohumoral activation in nonischemic heart failure. At present, the findings of stem cell therapy for heart failure are not consistent. Given the wide spectrum of heart failure, it may be difficult to define a uniform stem cell therapy for all subsets of patients;instead, future stem cell therapeutic strategies should aim for a more individualized approach by establishing optimal stem cell type, dose, delivery method and frequency of administration for different patients at different disease stages.
