摘 要:目的采用扩散张量成像(DTI)及基于纤维束追踪空间统计分析(TBSS)研究平原与高原健康成人的脑白质微细结构差异。方法对16名高原健康成人和与之年龄、受教育程度相匹配的20名平原健康成人行DTI检查,应用TBSS方法分析两组受检者脑白质纤维DTI的相关参数:各向异性分数(FA)、轴向扩散系数(AD)、平行扩散系数(RD)、平均扩散系数(MD)。对两组各项参数进行比较。结果与平原组比较,高原组在胼胝体体部、胼胝体压部、胼胝体膝部、两侧上纵束、两侧上放射冠、右侧内囊前肢、内囊豆状核后肢、右侧丘脑辐射线(包括视辐射线)的AD值显著增高(P<0.05,FWE校正);MD值显著升高的脑区是胼胝体体部、右侧上纵束、右侧上放射冠、右侧内囊前肢(P<0.05,FWE校正);RD值显著升高的脑区是胼胝体体部、右侧上纵束。结论通过DTI及TBSS分析,高原健康成人相对于平原健康成人脑白质纤维存在差异。Objective To study the cerebral white matter microstructure change between normal high altitude adults( HA group) and normal sea level adults( SL group) through Diffusion tensor imaging( DTI) and Tract-Based Spatial Statistics( TBSS). Methods Brain MRI and DTI scanning were performed in 16 HA group and 20 SL group with matched age and education level to get the DTI imaging. DTl parameters of WM,including fractional anisotropy( FA),axial diffusivity( AD),radial diffusivity( RD) and Mean diffusivity( MD). Then compare these four DTl parameters of the two groups with TBSS. Results AD increased in Body of corpus callosum,Splenium of corpus callosum,Genu of corpus callosum,bilateral Superior longitudinal fasciculus,bilateral Superior corona radiata,Right anterior limb of internal capsule,Left retrolenticular part of internal capsule,Right posterior thalamic radiation( include optic radiation) while increased RD values mainly located in Body of corpus callosum,Right superior longitudinal fasciculus,Right superior corona radiata,Right anterior limb of internal capsule. On MD maps,the increase was found in the Body of corpus callosum as well as the right superior longitudinal fasciculus and There were no significant differences of FA in every region between the two groups( all P〉0. 05,FEW correction). Conclusion The changes of WM microstructures were found in HA group when compared with SL group,which might reflect the deficit or degeneration of myelination to offset chronic hypoxia.
