直肠高分辨MRI结合腔内对比剂的应用对直肠癌术前诊断的价值

摘　　要：目的探讨应用腔内对比剂的直肠高分辨MRI对直肠癌术前诊断的价值。方法搜集2014年10月至2015年12月期间本院有临床相关症状且经病理证实的直肠癌52例患者的病例资料,所有患者术前均行直肠高分辨MRI及应用直肠腔内对比剂的高分辨MRI,并与术后病理结果作对照分析。结果 52例中,p T1 12例,p T2 27例,p T3 13例。高分辨MRI对直肠癌T分期的符合率为75%,Kappa值为0.56,P<0.05;采用直肠腔内对比剂的高分辨MRI能更清晰地显示直肠腔内的较小或多个病灶,对直肠癌T分期的符合率为85%,Kappa值为0.75,P<0.05。结论高分辨MRI结合直肠腔内对比剂的应用对直肠癌术前诊断及T分期有较高的诊断价值,能更清晰地显示直肠腔内的较小或多个的病灶。Objective To explore the clinical application of high resolution magnetic resonance imaging（ HR-MRI）with rectal contrast material in the preoperative diagnosis of rectal cancer. Methods 52 patients with pathologically confirmed rectal cancer were scanned using both the routine HR-MRI and the HR-MRI with rectal contrast material,and then the results were compared with the postoperative pathology. Results The pathology reports revealed that,out of the52 cases of rectal cancer,there were 12 p T1 cases of cancer,27 p T2 cancers,and 13 p T3 cancers. For the T staging,the routine HR-MRI determined an accuracy of 75%,Kappa = 0. 56,P〈0. 05 and the HR-MRI with the rectal contrast material showed an accuracy of 85%,Kappa = 0. 75,P〈0. 05. Conclusion The HR-MRI with rectal contrast material could provide some insight into the preoperative diagnosis of rectal cancer and its T staging,it could be more clearly displayed in the smaller or multiple lesions in the rectum cavity.
