脑卒中后抑郁不同中医证型情绪测量分析

摘　　要：目的:探讨脑卒中后抑郁(PSD)患者常见5种中医证候的情绪特征。方法:选择脑卒中中经络急性期以及恢复期并发抑郁患者,并选取脑卒中非抑郁患者为对照组,采用中医肝脏象情绪量表、HAMD、HAMA对各组进行情绪测量。结果:脑卒中患者中,以阴虚风动证PSD发病率为最高,PSD患者中医肝脏象情绪量表、HAMD、HAMA测值明显高于对照组(P<0.01);PSD之中医5证型比较,阴虚风动证、风痰火亢证中医肝脏象情绪量表总分及抑郁维度、焦虑维度积分高于其他证型,尤显著高于痰热腑实证(P<0.01)。结论:PSD患者有明显的焦虑及抑郁情绪,其5证型之间,阴虚风动证、风痰火亢证的焦虑、抑郁程度均高于其它证型。Objective ： To investigate the emotional characteristics of five kinds of syndromes in the post - stroke depression （PSD） patients. Methods ： The patients who were in the acute phase and convalescence of stroke involving the meridians and collaterals were selected as objects of observation and the control group were the patients who were stroke without depression. All the patients were assessed by using Emotions Rating Scale for Ganzangxiang of traditional Chinese medicine（ERSG） ,Hamilton anxiety scale （HAMA） and Hamilton depression scale （HAMD）. Results： The wind formation from Yin deficiency syndrome was the highest on the morbidity of PSD in the stroke patients. The accumulating scores of three dimensions in ERSG,HAMD and HAMA of PSD were higher significantly than that of controls（P 〈 0.01 ）. The wind formation from Yin deficiency syndrome and the wind - phlegm and hyperactive liver fire syndrome were higher than that of other syndromes, especially higher than phlegm - heat accumulation syndrome （P 〈 0. 01 ）. Conclusion ： PSD patients have marked anxiety and depression characteristics. The degree of depression and anxiety in patients with wind for- mation from Yin deficiency syndrome and the wind - phlegm and hyperactive liver fire syndrome were higher than that of patients with other syndromes, especially phlegmheat accumulation.
