肩贞、天宗及秉风脉冲射频热凝配合中药治疗慢性肩痛的效果观察

摘　　要：目的:观察肩贞、天宗及秉风脉冲射频热凝配合中药治疗慢性肩痛的效果。方法:128例慢性肩痛患者为研究对象。对照组(65例)给予洛索洛芬钠片口服及得宝松1 m L+2%利多卡因1.5 m L+0.9%氯化钠7.5m L痛点注射。观察组(63例)给予肩贞、天宗及秉风脉冲射频热凝治疗及自拟活血化瘀方口服。采用FMA评分、肩关节活动度(ROM)、改良Barthel指数比较两组治疗前后肩部活动功能变化,根据疼痛视觉模拟评分(VAS)、FMA评分比较两组效果。比较两组患者全血黏度、全血还原黏度等。结果:治疗后,观察组运动功能障碍情况、日常生活能力、肩部活动功能、生活质量及血液流变学相较于治疗前均显著变化(P<0.05),且优于对照组(P<0.05)。观察组有效率为88.9%、不良反应发生率为3.2%,对照组分别为66.2%、15.4%,差异均有显著性(P<0.05)。结论:脉冲射频热凝疗法与中药内服治疗相结合用于慢性肩痛有助于缓解疼痛状态、改善患者肩部活动功能及生活质量,且安全性高。Objective ： To observe the effect of pulse radiofrequency thermocoagulation at Jianzhen （ SI9 ）, Tianzong （SI11 ） and Bingfeng（SI12） and Chinese drugs on chronic shoulder pain. Methods： A total of 128 cases of chronic shoulder pain patients were studied. The control group （65 cases） were treated with loxoprofen sodium tablet and Diprospan 1 mL ＋2% lidocaine 1.5 mL ＋0.9% sodium chloride 7.5 mL by pain points injection. The observation group （63 cases） were given pulse radiofrequency thermocoagulation at Jianzhen （SI9）, Tianzong（ SI11 ） and Bingfeng（SI12） and self - made Huoxue Huayu Formula orally. The FMA score, shoulder joint activity （ROM） and modified Barthel index changes between two groups before and after treatment were used to evaluate the shoulder function. According to the visual ana- logue scale （VAS） and FMA score,we compared the two groups＇ blood viscosity and reduced blood viscosity. Results： After treatment, the observation group＇s movement dysfunction, activities of daily living （ADL）, shoulder function, quality of life and blood rheology were significantly changed （ P 〈 0. 05 ） and superior to those of the control group （ P 〈 0. 05 ）. The efficiency of the observation group was 88.9% and the incidence rate of adverse reaction 3.2%. The control group＇ s were 66.2% and 15.4% , respectively. There were significant differences （ P 〈0. 05 ）. Conclusion ： The pulse radiofrequency thermoeoagulation therapy combined with traditional Chinese medicine treatment for chronic shoulder pain helps to relieve pains and improves the function and quality of life, with high safety.
