经大隐静脉PICC置管在成人肿瘤化疗患者中的应用

摘　　要：目的探讨经大隐静脉PICC置管应用于成人肿瘤化疗患者的可行性及其效果。方法回顾性分析2012年1月至2015年12月56例肿瘤化疗患者的临床资料,其中31例经股静脉PICC置管作为对照组,25例经大隐静脉PICC置管作为观察组。比较两组一次穿刺成功率﹑下腔静脉一次到位率、术中出血量、术后疼痛和并发症情况。结果观察组PICC置管一次穿刺成功率明显高于对照组(P<0.05);术中出血量和术后疼痛评分明显优于对照组(P<0.05);两组下腔静脉一次到位率和并发症比较差异无统计学意义(P>0.05)。结论经大隐静脉PICC置管用于成人肿瘤化疗患者安全、可靠,可作为无法经上肢PICC置管的可靠替代方法。Objective To investigate the feasibility and efficacy of saphenous vein placed PICC catheterization in adult patients with tumor chemotherapy. Methods The clinical data of 56 patients with tumor chemotherapy from January 2012 to December 2015 were retrospectively analyzed. The control group included 31 cases of femoral vein placed PICC catheterization, 25 cases of saphenous vein placed PICC catheterization were in the observation group. The success rate of puncture, the first puncture arrival rate of inferior vena cava, the amount of bleeding, postoperative pain and complications were compared between the two groups. Results The success rate of puncture in the observation group was significantly higher than that of the control group （P〈0.05）, the amount of bleeding and postoperative pain score were significantly better than those in the control group （P〈0.05）. There were no significant difference in the first puncture arrival rate of inferior vena cava and the complications in the two groups （P〉0.05）. Conclusion Saphenous vein placed PICC catheterization in adult patients with tumor chemotherapy is safety and reliable, which could be used as the alternative PICC catheterization.
