摘 要:目的 探讨茵栀黄联合更昔洛韦治疗早产儿巨细胞病毒肝炎的临床疗效。方法 将 100 例巨细胞病毒肝炎早产儿随机分为治疗组和对照组。2 组均给予更昔洛韦静脉泵入(5 mg/kg,Q12 h)及复方甘草酸苷保肝治疗。治疗组加用茵栀黄口服液口服(5 mL/次,Q8 h)。疗程 2 周。观察 2 组患儿治疗前后血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、直接胆红素(DBIL)、γ-谷氨酸转肽酶(γ-GT)、血清总胆汁酸(TBA)、巨细胞病毒-DNA 定量(CMV-DNA)等指标变化。结果 治疗 2 周后,与对照组比较,治疗组血清 ALT、γ-GT、TBIL、DBIL、TBA 显著降低,差异均有统计学意义(q =4.286、4.745、5.397,4.875,4.953,P均〈0.05);治疗组 CMV-DNA 转阴 35 例(71.43%),对照组转阴 25 例(51.0%),差异有统计学意义(χ2=5.229,P = 0.022)。 结论 应用茵栀黄联合更昔洛韦治疗早产儿巨细胞病毒肝炎,在保肝、降黄、利胆方面均具有显著疗效。Objective To evaluate the clinical efficacy of Yinzhihuang injection combined with ganciclovir in the treatment of premature infants with cytomegalovirus hepatitis. Methods A total of 100 cases of cytomegalovirus hepatitis in premature infants were recruited for the study and randomly divided into treatment group and control group. Both of the two groups received gancielovir 5 mg/kg Q12 h and intravenous infusion of compound glycyrrhizin in the treatment of liver. In addition, the treatment group was treated with Yinzhihuang oral liquid 5 mUtime Q8 h. The changes of serum ALT, AST, --GT, TBIL, DBIL, TBA, CMV-DNA and other indexes were observed before and 2 weeks after treatment in two groups. Results After 2 weeks of treatment, serum ALT, γ-GT, TBIL, DBIL, TBA decreased significantly in the treatment group compared with control group, and the differences were statistically significant (q = 4.286,4.745,5.397,4.875,4.953, P 〈 0.05) ;there were 35 cases (71.43% of the treatment group) exhibited CMV-DNA negative after treatment, which was 51% in control group (25 cases), the difference was significant between two groups (χ^2=5.229, P = 0.022). Conclusion There was combined effects of ganciclovir in the treatment of premature infants with cy- tomegalovims hepatitis, which protects liver, reduces jaundice, promotes bile flow.
