摘　　要：目的探讨心包剥离术对老年慢性缩窄性心包炎心钠素与内皮素变化的影响。方法收集慢性缩窄性心包炎患者66例,区组随机化法分组为对照组和观察组,每组各33例,患者均给予相应治疗,对照组给予相应强心、扩血管药物,观察组患者行心包剥离术,治疗结束后,对所有患者的心钠素(ANF)、内皮素(ET)、血压以及临床治疗效果进行检测并比较。结果与治疗前相比,治疗后两组患者ANF、ET、舒张压以及脉压水平均下降,收缩压水平升高(P<0.05);与对照组相比,观察组患者ANF、ET、舒张压以及脉压水平较低,收缩压水平较高,射血分数EF值升高(P<0.05);观察组患者术前术后的心功能评级的差异有统计学意义;观察组患者治疗总有效率较高(P<0.05)。结论心包剥离术能够降低慢性缩窄性心包炎患者ANF、ET、舒张压以及脉压水平,升高收缩压水平,临床疗效较好,对临床有指导意义。Objective To investigate the effect of pericardiectomy on changes in atrial natriuretic peptide and endothelin of elderly pa-tients with chronic constrictive pericarditis.Methods Sixty-six cases of chronic constrictive pericarditis were randomized into observa-tion and control group,with 33 cases in each group.Patients in the control group were given cardiotonic vasodilators,while those in the observation group underwent pericardiectomy.After treatment,all patients′ANF,ET,blood pressure and clinical therapeutic effect were detected and compared.Results Compared with before treatment,ANF,ET,diastolic pressure and pulse pressure levels after treatment were decreased in both groups,while systolic blood pressure in both groups was elevated（P 〈0.05）.Compared with the control group, ANF,ET,diastolic pressure and pulse pressure were lower,the systolic blood pressure levels and ejection fractions were higher in pa-tients of the experimental group （P 〈0.05）.There was significant difference in ranking of heart function before and after treatment.The total efficiency is higher in the observation group（P 〈0.05）.Conclusions Pericardiectomy can reduce ANF,ET,diastolic blood pres-sure and pulse pressure of patients with chronic constrictive pericarditis and elevated their systolic blood pressure,the clinical curative effect of which was better and there is guiding significance in clinic.
