摘 要:目的了解影响儿童注意缺陷多动障碍(ADHD)发生的多巴胺羟化酶(DβH)基因特定位点多态性及相关环境影响因素间的关系,为综合干预ADHD的发生提供科学依据。方法选取包头市市区及郊区各1所小学,对全体在校生进行流行病学调查,筛查并诊断出ADHD儿童104名,选取同校其他同性别年龄相差〈2岁的相互间无血缘关系的健康儿童104名为对照,分析环境影响因素、DβH基因特定位点多态性及二者之间交互作用与ADHD的关系。结果 Logistic回归分析显示,年级(OR=0.622)和家庭知识性(OR=0.747)是ADHD发生的保护因素,而父亲惩罚、严厉(OR=1.138)和父亲偏爱被试(OR=1.140)是危险因素,DβH5'侧翼区-1021C/T-CC基因型是ADHD发生的危险因素(OR=2.040)(P值均〈0.05)。基因多态性检测显示,ADHD组与对照组DβH5'侧翼区-1021C/T基因型分布比较差异无统计学意义(χ^22.297,P=0.130),两组等位基因频率比较差异无统计学意义(χ^20.019,P=0.780)。基因-环境交互作用结果显示,父亲惩罚、严厉与DβH5'侧翼区-1021C/T-CC基因型发生存在负向交互作用(r=0.357,OReg=1.047,P=0.027),父亲偏爱被试与DβH5'侧翼区-1021C/T-CC基因型发生存在负向交互作用(r=0.557,OReg=1.076,P=0.035)。结论特定位点基因型与特定环境因素间的交互作用可能影响ADHD的发生。Objective To examine the interaction of DβH gene polymorphisms and environment on the occurrence of ADHD among children. Methods Two representative primary schools in Baotou were selected, and 104 students with ADHD were screened and diagnosed. Healthy control( n = 104) were matched with case group by sex and age. DβH gene polymorphism, and its interactive with environment was explored with ADHD. Results Logistic regression analysis showed that grades( OR = 0.622) and family knowledge( OR = 0.747) were protective to ADHD( P0.05), parental style of punishment and harsh( OR = 1.138) and preference( OR = 1.140) were risk factors of ADHD, respectively. DβH5' flanking region-1021- CC genotype increased the risk of ADHD( OR = 2.040, P0.05). There was no significant difference between the two groups in DβH5' flanking region of-1021 C / T genotype( χ^2 2.297, P = 0.130), allele frequency( χ^2 0.019, P = 0.780). Parenting style of punishment / harsh and DβH5' flanking region-1021-CC genotype had significant interaction( r = 0.357, OReg = 1.047, P = 0.027). Parenting style of preference and DβH5' flanking region-1021-CC genotype had significant interaction( r = 0.557, OReg = 1.076, P = 0.035). Conclusion The interaction between genotype and specific environmental factors had influences on risk of ADHD.
