摘  要:目的通过检测急性ST段抬高型心肌梗死(STEMI)患者治疗前后血清内脏脂肪素(内脂素)、基质金属蛋白酶-9(MMP-9)、超敏C反应蛋白(hs-CRP)水平,探讨丹参多酚酸盐治疗STEMI中的机制。方法采用随机分组的方法,将100例STEMI患者随机分为2组,对照组50例和治疗组50例,测定患者治疗前后血清中内脂素、MMP-9、hs-CRP水平,另外选取50例非冠心病患者作为参考值组,采集血清中以上3种指标作为基础参考值。结果对照组及治疗组治疗后血清内脂素、MMP-9、hs-CRP水平均下降(P〈0.05),治疗组内脂素、MMP-9、hs-CRP降低幅度明显大于对照组(P〈0.05),且内脂素与MMP-9(r=0.583,P〈0.01)、hs-CRP(r=0.674,P〈0.01)均呈显著正相关。结论丹参多酚酸盐能显著降低急性STEMI患者血清中内脂素、MMP-9、hs-CRP,可能具有抗炎症反应、促进梗死周边心肌修复、抑制心室重构、改善预后等作用。Objective To investigate the treatment mechanism of depside salt from salvia miltior- rhiza in STEMI through detecting the levels of visfatin, matrix metalloproteinases-9 ( MMP-9 ) and hyper- sensitive C-reactive protein (hs-CRP) in serum before and after therapy of ST-segment elevation myocar- dial infarction(STEMI) patients. Methods 100 STEMI patients were divided into two groups randomly, 50 patients in control group and 50 patients in experimental group. Levels of visfatin, MMP-9 and hs-CRP in serum were detected. The patients without coronary heart disease served as another group and the levels of visfatin,MMP-9 and hs-CRP in this group were used as a basic reference value. Results The levels of visfatin, MMP-9 and hs-CRP decreased( P 〈 0.05 ) in the control group and experimental group after treat- ment. The decreasing range of experimental group were higher than that in the control group ( P 〈 0.05 ). Visfatin presented a notable positive correlation with MMP-9 (r = 0.583 ,P 〈 0.01 ), and hs-CRP( r = 0.674, P 〈 0.01 ). Conclusion The depside salt from salvia miltiorrhiza can reduce the levels of visfatin, MMP-9 and hs-CRP in STEMI patients' serum. It has the abilities of anti-inflammatory, promotes myocardial repair in infarct periphery,inhibits ventricular remodeling and improves prognosis.
