布地奈德福莫特罗联合乙酰半胱氨酸对稳定期中重度慢性阻塞性肺疾病患者氧化应激的影响

摘　　要：目的探讨布地奈德福莫特罗（信必可都保）联合乙酰半胱氨酸对稳定期中重度慢性阻塞性肺疾病（COPD）患者机体氧化应激的影响。方法选择80例稳定期中重度COPD患者，随机分为试验组和对照组，每组各40例。在常规治疗基础上，试验组给予布地奈德福莫特罗联合乙酰半胱氨酸泡腾片治疗，对照组仅给予布地奈德福莫特罗单药治疗，疗程8周。两组患者在治疗前后均行肺功能检查，并检测血清中谷胱甘肽过氧化物酶（GSH-PX）、丙二醛（MDA）和超氧化物歧化酶（SOD）的含量，进行比较分析。结果（1）治疗后，两组患者的肺功能指标较治疗前均有改善，且试验组改善更加明显（P〈0．05）；（2）治疗后，两组患者的血清GSH-PX、SOD均高于其治疗前水平（P〈0．05），MDA均低于治疗前水平（P〈0．05），且试验组变化更为显著，与对照组比较差异有统计学意义（P〈0．05）。结论相比单用布地条德福莫特罗治疗，长期联合使用乙酰半胱氨酸，能有效调节COPD患者机体氧化一抗氧化平衡，延缓肺功能下降，具有临床推广价值。Objective To investigate the effect of Symbicort Turbuhaler combined with acetylcysteine on the oxidative stress in stable phase of moderate and severe patients with chronic obstructive pulmonary disease（COPD）. Methods 80 patients with stable phase of moderate and severe COPD were randomly divided into experimental group and the control group, with 40 cases in each group. On the basis of conventional treatment, experimental group was given Symbicort Turbuhaler combined with acetylcysteine while the control group was only given Symbicort Turbuhaler with the treatment period of 8 weeks. Lung function, the levels of glutathione peroxidase （ GSH-PX ）, malondialdehyde （ MDA ） and superoxide dismutase（SOD） were monitored and analyzed in two groups before and ＇after treatment. Results （1）After 6 weeks of treatment, the lung functions two groups of patients improved and the effect of the experimental group improved more obvious （ P 〈 0.05 ） ; （ 2 ） After treatment, two groups of patients with serum GSH-PX and SOD activity were higher than those before the treatment （ P 〈 0.05 ）, MDA content were lower than that before treatment（P 〈0.05 ）. And experimental group patients serum GSH-PX, SOD activity was higher than the control group （ P 〈 0. 05 ）, the MDA content is lower than the control group （ P 〈 0. 05 ）. Conclusion Compared to the use of single drug treatment with Symbicort Turbuhaler treatment,combination of acetylcysteine for a long time, can effectively adjust the oxidation-antioxidant balance in patients with COPD, delay lung function decline, should be popularized in clinical.
