缩宫素和卡贝缩宫素对硬膜外麻醉下剖宫产产妇平均动脉压和心率的影响比较

摘　　要：目的:对比硬膜外麻醉下静脉注射缩宫素和卡贝缩宫素对剖宫产产妇平均动脉压和心率的影响。方法:选择拟行剖宫产的产妇90例随机分为3组,当胎儿头及肩膀娩出后,安慰剂组静脉注射5 m L 0.9%氯化钠溶液,缩宫素组注射5 m L缩宫素溶液(1.0 U/m L),卡贝缩宫素组注射5 m L卡贝缩宫素溶液(浓度20μg/m L)。观察记录3组用药后不同时间点产妇的平均动脉压、心率和新生儿Apgar评分及不良反应。结果:缩宫素组和卡贝缩宫素组的产妇平均动脉压显著下降,与安慰剂组比较差异有统计学意义(P<0.05),且缩宫素组和卡贝缩宫素组间差异无统计学意义。3组产妇心率均有增加,与安慰剂组相比,给药2 min后缩宫素组和卡贝缩宫素组心率增加有统计学意义(P<0.05)。给药5 min后安慰剂组和缩宫素组的心率恢复至基础值,而卡贝缩宫素组心率仍高于基础值。三组新生儿Apgar评分、不良反应差异无统计学意义。结论:硬膜外麻醉下静脉注射5 U缩宫素和100μg卡贝缩宫素对剖宫产产妇的平均动脉压和心率影响是相当的。Objective To compare the effects of oxytocin and carbetocin on the parturients＂ mean arterial pressure and heart rate in the cesarean delivery with epidural anesthesia. Methods Ninety parturients scheduled for cesarean were randomly di- vided into three groups ,control group, oxytocin group and carbetocin group.When fetal head and shoulder expulsion, parturi- ents of three groups were given 5 mL 0.9% sodium chloride solution, 1.0 U/mL oxytocin and 20 μg/mL carbetocin,respec- tively.Then the mean arterial pressure and heart rate of parturients at different time points,the Apgar score of neonatus, and the adverse reaction were recorded~ ResuIt$Compared with control group, the mean arterial pressure of parturients were sig- nificantly decreased in oxytocin group and carbetocin group（P〈0.05） ,but which had no significant difference between oxy- tocin group and carbetocin group.The heart rate was increased in all the parturients, whereas there was a significantly in- crease in oxytocin group and carbetocin group compared with control group at 2 min after administration（P〈0.05） .The heart rate was recovered to basal level in both control group and oxyctocin group, but which was still, higher than basal level in carbetocin group.The Apgar scores of neonatus and the incidences of adverse reaction had no significant difference among three groups.Conclusion In the cesarean delivery with epidural anesthesia,the effects of oxytocin with dose of 5 U and car- betocin with 100 μg on the parturients＂ mean arterial pressure and heart rate were equivalent.
