摘  要:目的:采用Meta分析的方法比较臭氧(O3)关节腔内注射联合糖皮质激素(GC)痛点封闭与单一GC痛点封闭治疗肩周炎的有效性。方法:计算机检索Pub Med、Cochrane Library(2015年第11期)、Embase、CNKI、CBM、VIP及Wan Fang数据库中相关的临床随机对照试验(RCT),截止日期为2015年11月24日。2名研究者按纳入和排除标准独立筛选文献、提取数据和评价文献质量,采用Rev Man 5.3软件进行统计分析。结果:共纳入7个RCT,合计肩周炎患者698例。Meta分析结果显示:O3关节腔内注射联合GC痛点封闭于单一GC痛点封闭相比,在治疗有效率方面(OR=5.82,95%CI=3.18~10.64)和缓解肩关节疼痛方面(MD=-2.06,95%CI=-2.41^-1.72)差异有统计学意义。敏感性分析显示结果稳定性较好,漏斗图显示无明显发表偏倚。结论:当前的证据表明O3关节腔内注射联合GC痛点封闭在治疗有效率和减轻患者肩关节疼痛方面优于单一GC痛点封闭。基于现有研究缺陷,本研究结论需开展大量高质量RCT予以证实。Objective To compare the effectiveness of Ozone ( 03 ) intra- articular injection combining with glucocorticoid (GC) periarticular blocking( 03 + GC ) with GC periartieular blocking alone for frozen shoulder by using meta-analysis. Methods We searched the PubMed, Cochrane Library( November 11,2015), Embase, CNKI, CBM, VIP and WanFang data- bases for relevant clinical randomized controlled trials (RCT). Two researchers selected the studies, extracted information and assessed studies" quality independently.Examined the studies and refined useful information, and then did the evaluation and meta-analyzed by using RevMan 5.3 software. Results Total 7 RCTs with 698 patients were included.The pooled results showed that there were significant differences between 03 +GC and GC alone in treated efficacy( OR = 5.82,95%CI = 3.18 ~ 10.64) and releasing the shoulder joints pain( MD=-2.06,95%CI=-2.41--1.72).Conclusion Current evidence suggest that 03 combining with GC is superior to GC therapy alone for frozen shoulder.Due to the limitation of the present studies, our findings need to be verified further by more large-scale and well-designed RCTs.
