摘 要:背景:中药威灵仙化学成分多样,药理作用广泛。前期研究显示威灵仙能有效干预实验性肝纤维化。目的:研究威灵仙总皂苷对实验性非酒精性脂肪性肝炎(NASH)大鼠模型血清炎症细胞因子和肝脏病理的干预作用。方法:以高脂饲料饲养16周诱导实验性NASH大鼠模型,分别予多烯磷脂酰胆碱和威灵仙总皂苷灌胃干预8周,同时设置不予药物干预的模型组和以普通饲料饲养的空白组。以ELISA法检测血清白细胞介素-6(IL-6)、IL-17、肿瘤坏死因子-α(TNF-α)水平,以非酒精性脂肪性肝病(NAFLD)活动度积分评估肝脏脂肪变性分度和炎症分级。结果:经8周药物干预后,威灵仙总皂苷组血清IL-6、IL-17、TNF-α水平显著低于模型组(P<0.01),与空白组间差异无统计学意义(P>0.05),肝脏脂肪变性分度和炎症分级亦较模型组显著减轻(P<0.05),与临床常用保肝药物多烯磷脂酰胆碱的作用无明显差异(P>0.05)。结论:威灵仙总皂苷可有效改善实验性NASH大鼠模型的肝脏脂肪变性和炎症损伤,降低血清炎症细胞因子水平,疗效与多烯磷脂酰胆碱相当。其机制可能与调节肝脏免疫、对抗肝脏氧化应激、改善胰岛素抵抗等有关。Radix clematidis is a Chinese herbal medicine with complex chemical components and broad pharmacological activities. It has been demonstrated that Radix clematidis is effective in treating experimental liver fibrosis. Aims: To investigate the intervention effect of total saponins of Radix clematidis on serum inflammatory cytokines and liver pathology in rats with experimental non-alcoholic steatohepatitis (NASH). Methods: Sprague-Dawley rats were fed with high fat diet for 16 weeks to induce experimental NASH and then received polyene phosphatidyl choline or total saponins of Radix clematidis intragastrically for 8 weeks. Rats fed with high fat diet without drug intervention and rats fed with normal diet were served as NASH control and blank control, respectively. Serum levels of interleukin-6 (IL-6) , IL-17 and tumor necrosis factor-α (TNF-α) were measured by ELISA method, the hepatic steatosis and inflammation were evaluated by using non-alcoholic fatty liver disease (NAFLD) activity score. Results: After 8 weeks intervention, serum levels of IL-6, IL-17 and TNF-α were significantly decreased in total saponins group than in NASH control group ( P 〈 0.01 ) , and no significant difference was seen between total saponins group and blank control group ( P 〉 0.05 ). Severity of hepatic steatosis and inflammation were also reduced markedly in total saponins group ( P 〈 0.05 ). The efficacy of total saponins of Radix clematidis was similar to that of polyene phosphatidyl choline, a commonly used hepatoprotective agent ( P 〉 0.05 ). Conclusions: Total saponins of Radix clematidis is effective for ameliorating hepatic steatosis and inflammatory injury and reducing serum inflammatory cytokines in rats with experimental NASH, the efficacy is equal to polyene phosphatidyl choline. Total saponins of Radix clematidis may exerts the therapeutic effect on NASH by its immunomodulation, antioxidant and anti-insulin resistance activities.
