摘  要:目的本研究通过文献检索分析,评估维生素C在预防四肢创伤或围手术期发生复杂性区域疼痛综合征(CRPS)的有效性。方法用于文献检索的数据库有:Ovid EMBASE,Ovid MEDLINE,CINAHL,the Cochrane Database和中国知网数据库(CNKI),从中检索关于围手术期患者每日服用不少于500mg维生素CRPS发病率的文献。结果根据纳入和排除标准对所检索的文献进行筛选,选中5篇文献进行Meta分析,其中1篇是关于足踝创伤或手术患者,另4篇是关于上肢创伤患者。所有5篇文献的研究均满足了除干预措施外的最小异质性(Tau^2=0.00)。最终Meta分析显示,在患肢受伤或手术后口服维生素C至少500mg,疗程50d,比值比(OR)为0.20[95%CI=(0.12,0.34)]。结论创伤或术后当天开始服用维生素C可以有效预防CRPS的发生。Objective To evaluate the prophylactic effect of Vitamin C on the complex regional pain syndrome( CRPS ) after extremities trauma or surgery. Methods The databases used for this review included : Ovid MEDLINE, CINAHL,Ovid EMBASE, the Cochrane Database and CNKI. The literatures about peri-operative patients with daily doses of no less than 500mg of vitamin for preventing CRPS were retrieved. Results After screening with inclusion and exclusion criteria,we selected 4 studies which were about upper extremities trauma and 1 study which was about foot and ankle surgery. All the 5 studies met the minimum heterogeneity without interventions (Tau^2 = 0. 00). The quantitative analysis showed an odds ratio of 0. 21 (95% confidence interval =0. 12, 0. 34) when daily vitamin C of at least 500 mg was given immediately after the extremity surgery or injury and was continuously applied for the following 50 days. Conclusion Taking vitamin C immediately after extremity trauma or surgery can effec- tively prevent the occurrence of CRPS.
