摘 要:目的探讨不同营养干预对肌肉衰减综合征患者肌力和肌含量的影响。方法选择91例就诊的肌肉衰减综合征患者,随机分为三组(宣教+全营养配方粉组、宣教+蛋白粉组、宣教组),分别在干预前、干预2月、干预3月检测体重、握力、小腿围、步速及5次起坐时间、四肢肌肉含量和躯干肌肉含量。结果三组患者干预前、干预2月和干预3月体重、握力、小腿围、步速及5次起坐实验时间比较,差异均无统计学意义(F分别=1.07、1.57、1.14、0.26、0.87、1.64、1.35、1.50、0.48、0.79、0.15、1.34、0.75、0.07、0.44,P均>0.05)。组内比较显示,宣教+全营养配方粉组患者步速和5次起坐试验在干预2月和干预3月明显高于干预前(P均<0.05),宣教+蛋白组患者步速在干预2月和干预3月明显高于对照组,5次起坐试验干预3月后明显低于干预前(P均<0.05)。三组患者在干预前和干预后的四肢肌肉含量和躯干肌肉含量比较,差异均无统计学意义(F分别=0.44、0.56、0.02、0.01,P均>0.05);三组患者干预后的四肢肌肉含量和躯干肌肉含量与同组干预前比较,差异均无统计学意义(t分别=1.12、-0.29、-0.03、0.02、-1.84、-0.75,P均>0.05)。结论适当增加全营养配方粉和蛋白粉均可以改善肌肉衰减综合征人群步速和5次起坐时间,建议老年肌肉衰减综合征人群饮食基础上适当补充能量和蛋白质的摄入。Objective To investigate the effect of different nutritional intervention on muscle strength and content in patients with sarcopenia. Methods A total of 91 patients with sarcopenia were collected and randomly divided into three groups(full nutritional products group,education+protein group,education group).The weight,grip,calf circumference,pace,5th-ups were compared before research,intervene for 2 months and 3months. Results The weight,grip,calf circumference,pace,5th-ups comparison in three groups had no difference(F=1.07,1.57,1.14,0.26,0.87,1.64,1.35,1.50,0.48,0.79,0.15,1.34,0.75,0.07,0.44,P 0.05). After 2 and 3 months of intervention,the pace and 5th-ups of full nutritional products group were significantly higher than before intervention(P0.05),the pace of education+protein group were significantly higher than before intervention and5th-ups were lower than before intervention(P 0.05). The limbs and trunk muscles were no significant differences in three groups(F=0.44,0.56,0.02,0.01,P0.05), also before and after intervention were no significant differences(t=1.12,-0.29,-0.03,0.02,-1.84,-0.75,P 0.05). Conclusion Appropriate increase in nutritionally complete formula powder and protein powder can improve less pace,and time-ups five times in sarcopenia,so suggest to appropriate supplemental energy and protein intake in elderly with sarcopenia on the basis of the diet.
