鼻咽癌致麻痹性斜视漏诊1例

摘　　要：麻痹性斜视是指由支配眼球运动的神经核、神经以及眼外肌本身麻痹所致的斜视。其典型特点是眼球运动障碍。属于非共同性斜视的一种。麻痹性斜视常见原因有炎症、血管性疾病、占位性疾病、先天异常、外伤等。鼻咽癌是指发生于鼻咽腔顶部和侧壁的恶性肿瘤，是我国高发恶性肿瘤之一，发病率为耳鼻咽喉恶性肿瘤之首。常见临床症状为鼻塞、涕中带血、耳闷堵感、听力下降、复视及头痛等。早期症状不典型，临床上易引起漏诊。本文就鼻咽癌以斜视为首发病症为例浅谈其发病机制，以便于今后眼科医师临床诊疗。The paralytic strabismus is often caused by paralysis of the nerve nucleus, nerve, or the muscle of the eye. Its typical characteristic is the obstacle of eye movement. It is one of the non-concomitant strabismus. The common cause of the paralysis of strabismus is inflammation, vascular disease, accounting for the disease, congenital abnormalities, trauma, etc. Nasopharyngeal carcinoma （NPC）is a malignant tumor that occurs at the top and the side wall of the nasopharyngeal cavity. It is one of the malignant tumors of high incidence in our country. The common clinical symptoms are nasal obstruction, bloody discharge, ear stuffy, sense of hearing loss,headache and diplopia. The early symptoms are not typical,and the clinical diagnosis is easy to be misdiagnosed. To help the clinical diagnosis and treatment in the future,we use one case to discuss the pathogenesis of paralytic strabismus caused by nasopharyngeal carcinoma, in which strabismus come as the first symptom of the NPC.
