Correlation between the TI-RADS Ultrasonographic Categories and the Tumor Biological Behaviour in Thyroid Carcinoma

摘  要:目的探讨甲状腺影像报告及数据系统(TI-RADS)超声指标与肿瘤生物学行为的相关性。方法将手术病理证实的103例甲状腺癌患者,共计108枚病灶的TI-RADS超声指标与肿瘤侵犯包膜或包膜外周围组织及淋巴结转移进行相关性分析。结果 (1)108枚病灶中颈部淋巴结转移56枚;淋巴结未转移52枚;癌组织侵犯包膜或侵及包膜外组织38枚;癌组织局限于腺体内70枚。(2)单因素和Logistic回归分析显示甲状腺癌≥2cm、结节内微钙化灶发生颈部淋巴结转移增多(P<0.05),以结节内微钙化灶预测更有价值。(3)单因素分析显示病灶≥2cm对预测甲状腺癌包膜或包膜外侵犯有较高价值(P<0.05)。结论 TI-RADS超声指标中结节大小和微钙化灶与肿瘤生物学行为具有一定的相关性,可根据甲状腺癌的超声表现初步预测其预后。Objective To investigate the correlation between ultrasonographic categories described by thyroid imaging reporting and data system(TI-RADS) and the tumor biological characteristic. Methods The ultrasonographic categories of 108 lesions in 103 patients and their tumor biological characteristic confirmed by pathology were analyzed. Results (1)In 108 examined nodules, 56 were metastasis cervical lymph node, 52 were not. 38 had extracapsular invasion and 70 had not. (2) Univariate and logistic regression analysis showed tumors greater than 2 cm in size and microcalcifications in the thyroid carcinoma had more positive cervical lymph nodes than other ultrasonographic categories of TI-RADS, especially the latter. (3)Univariate analysis showed tumors greater than 2 cm in size had more predictive value for the presence of an extrathyroid extension than other ultrasonographic categories of TI-RADS. Conclusions There is significant correlation of tumor size, presence of microcalcification on ultrasonography with thyroid tumor biological behaviour characteristic. According to the ultrasonographic findings of thyroid carcinoma, prognosis can be preliminary predicted.
