摘  要:目的 探讨年龄及乳腺超声类型对乳腺结节超声BI-RADS分类的影响。方法 回顾分析乳腺结节手术病例311例,判读其乳腺超声分型及乳腺结节的BI-RADS分类。以病理结果为金标准,按临床处理原则不同,将超声BI-RADS分类1~3纳入临床随访,视为阴性,BI-RADS分类4~5纳入临床干预,视为阳性。按照年龄段及乳腺超声类型分别统计乳腺结节超声BI-RADS分类的准确率,采用软件GraphPad Prism 5进行统计分析,P〈0.05为差异有显著性意义。结果 排除患者年龄因素,各型腺体结节超声BI-RADS分类的准确率总体上没有显著差异(P〈0.05)。腺体型乳腺结节超声BI-RADS分类的准确率60岁以下组明显高于60岁以上组(P〈0.01);腺纤维Ⅰ型及腺纤维Ⅱ型乳腺结节超声BI-RADS分类的准确率在各年龄组间没有显著差异(P〈0.05);脂肪型乳腺结节超声BI-RADS分类的准确率40岁以下组明显高于40岁以上组(P〈0.01)。结论 超声BI-RADS分类对各型乳腺结节的判读都有较高的临床符合率。年龄因素对腺体型及脂肪型乳腺的超声BI-RADS分类准确率有明显影响。Objective To investigate the influence of age and the ultrasonic type of breast on BI-RADS-US category for breast nodules. Methods The data of 311 cases with breast nodules treated by operation were analyzed retro spectively, and the ultrasonic type of breast and BI-RADS-US category of breast nodules were judged. According to the principle of clinical treatment, BI-RADS-US categories lto 3 were defined as negative and BI-RADS-US categories 4 to 5 were defined as positive. The pathological results were as the gold standard, and the accuracy rate of BI-RADS- US category for breast nodules in every ultrasonic type of breast at different ages was calculated, and Prism 5 statistical software was used for analysis. The probability (P value) of less than 0.05 was used as a cutoff point. Results As a whole, there was no significant difference on the accuracy of BI-RADS-US category for breast nodules among all the ultrasonic type of breast (P〈0. 05). The accuracy of BLRADS-US category for breast nodules in glandular type under the age of 60 was significantly higher than that of above 60 years old (P〈0.01) ; there were no significant differences in the accuracy of BI-RADS-US category for breast nodules in glandular fiber type Ⅰ and glandular fiber type Ⅱ among all age groups(P〈0.05). The accuracy of BI-RADS-US category for breast nodules in fat type under 40 years of age was significantly higher than that of above 40 years old group (P〈0. 01). Conclusions The accuracy of BI-RADS-US category for breast nodules in various types of breast had a higher clinical coincidence rate. Age had a significant influence on the accuracy of US-BI-RADS category for breast nodules in glandular type breast and fat type breast.
